Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between Oct. 25 and Oct. 27:

Springmill Stem Elementary, 1200 Nestor Drive, Mansfield, Oct. 25. Person in Charge (PIC) did not ensure employees are properly maintaining temperatures of TCS foods during hot and cold storage (critical). Tossed salad and Hobart cooler were both out of temperature. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Hobart cold holding unit was registering 54 degrees when opened. Temperatures of single portions of dairy milk measured at 46-47 degrees. Ready-to-go lunches in cooler relocated into different cooler. TCS foods not being cold held at the proper temperature (critical, corrected). TSC tossed salad on serving line was measured at 55 degrees. Listed item was removed from serving line to be discarded.

Lexington Court Care Center, 250 Delaware St., Lexington, Oct. 25. Handwashing sink is being used for purposes other than handwashing (critical, corrected during inspection). Observed a food employee filling glasses from the hand washing sink. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical repeat). Quat sanitizer at the three-compartment sink was dispensing around 100 ppm at the time of inspection. Correct by Nov. 1.

Panera Bread No. 4780, 2070 Walker Lake Road, Mansfield, Oct. 25. Food not properly protected from contamination by separation, packaging, and segregation (critical, corrected during inspection). Observed toppings on the front make line to be uncovered. TCS foods not being cold held at the proper temperature (critical, corrected). Observed sliced American, gouda, mozzarella cheeses, and arugula and spinach to have internal temperatures between 45-48 degrees in the back make line. Creamer at the coffee stations had internal temperature of 58 degrees. PIC voluntarily discarded items.

Richland Skyline Inc., 1320 N. Lexington Springmill Road, Mansfield, Oct. 25. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed small serving bowls in drawer near servers station to contain visible residue/dried spillage on inside of bowls. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical, corrected). Observed dishwasher to have a minimum temperature of 110 degrees. PIC adjusted the water heater to allow the dishwasher to reach the required 120 degrees.

Lex Lanes, Lex-Luray Enterprises Inc., 60 Ohio 97, Lexington, Oct. 26. PIC unable to demonstrate knowledge by having no critical violations. (critical). Food not properly protected from contamination by separation, packaging, and segregation (critical, corrected during inspection). Observed bottle of fireball to be opened with no cap. Equipment food-contact surfaces or utensils are unclean (critical repeat). Observed visible residue on the pop nozzles of the bar area. Clean by Nov. 2. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed no date marking on nacho cheese cups, American cheese and tomatoes. PIC voluntarily discarded nacho cheese cups and American cheese. The tomatoes were recently prepped and PIC labeled them with seven day discard. Improper storage of poisonous or toxic materials (critical, corrected). Observed bottle of Dawn degreaser and Clorox wipes stored over the ice bin and soda machine in the kitchen. Food contact surfaces not easily cleanable (critical). Observed broken handle of ice scoop rendering surface no longer smooth, durable and easily cleanable. Replace by Nov. 2.

Woodland Elementary, 460 Davis Road, Mansfield, Oct. 26. Food equipment surfaces not cleaned at required frequency (critical repeat, corrected during inspection). Worker measured broccoli without sanitizing food contact surface of probe thermometer. TCS foods not being cold held at the proper temperature (critical, corrected). Two-door Traulsen cooler had milk inside that measured 51 degrees. PIC discarded 30, one pint portions of milk.

Madison Early Childhood Learning Center, 1035 Grace St., Mansfield, Oct. 27. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed can opener blade with food residue on blade.

Nickel & Bean, 27 E. Main St., Lexington, Oct. 27. Missing and/or incomplete allergen declaration(s) (critical, corrected during inspection). Observed dried apricots to be without information that they contain sulphur dioxide. Place a placard with all ingredients on the basket for customer information. PIC pulled apricots from sale at time of inspection.

Outback Steakhouse, 820 Lexington Springmill Road, Mansfield, Oct. 27. Equipment food-contact surfaces or utensils are unclean (critical repeat, corrected during inspection). Observed food contact utensils (ladle, scoop and tongs) hanging on clean storage rack near hand sink in warewashing area to contain visible dried food residue.

Wiener King/Sunset Fast Foods Inc., 118 Lexington Ave., Mansfield, Oct. 27. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). Forms provided. Unsafe, adulterated or not honestly presented food not discarded or properly reconditioned (critical, corrected). Ketchup container in walk-in cooler has visible black build up. PIC voluntarily discarded. Ready-to-eat, TCS food not properly discarded when required (critical, corrected). Milk that had been opened week before was not discarded. PIC removed from service area to discard. Ready-to-eat, TCS food not properly discarded when required (critical, corrected). Sliced tomato in make table had been in for a week after slicing. PIC voluntarily discarded.

McDonald's Restaurant/DBA Bellville Arch No. 10933, 850 Ohio 97 W., Bellville, Oct. 27. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed tea in dispensers without lids/protection from contamination. Presence of live rodents (critical). Observed rodent droppings on back of counter, behind fountain beverage station in drive-thru area and behind coffee carafes behind front counter area. Facility does work with licensed pest control company and does have active measures in place. Remove all evidence of activity by Nov. 3. Improper temperature of wash solution in mechanical warewasher (critical, corrected). Observed temperature of mechanical warewashing machine to be 100 degrees during wash cycle. PIC had maintenance repair machine during inspection.

Little Blessings 'N More, LLC, 250 Glessner Ave., Mansfield, Oct. 27. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected during inspection). Observed cups of milk on a tray in the cooler that were not covered. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical, corrected). Bucket of bleach water in the three-compartment sink was at 0 ppm. PIC remade solution to a concentration of 100 ppm. Improper storage of poisonous or toxic materials (critical, corrected). Observed gallons of bleach stored on a shelf above packages of single use foam plates.

Lexington Child Development Center, 236 Otterbein Dr., Lexington, Oct. 27. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Observed no datemark on the American cheese. Direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed (critical, corrected). Observed no backflow device installed on mop sink. PIC removed hose connected to sink and discontinued use until proper backflow can be installed.
