Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between Aug. 17 and Aug. 22:

Bake My Day #2, 803 N. Lexington Springmill Road, Ontario, Aug. 17. Missing and/or incomplete allergen declaration(s) (critical, corrected during inspection). Observed chocolate pizzas for sale in customer self-service retail area missing allergen declaration. Person in charge (PIC) removed and placed behind counter.

Hong Kong Buffet, 331 N Lexington Springmill Road, Mansfield, Aug. 17. PIC unable to demonstrate knowledge by having no critical violations (critical repeat, corrected during inspection). PIC did not ensure employees are properly maintaining temperatures of TCS foods during hot and cold storage (critical repeat, corrected). Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected). Observed raw eggs stored below raw chicken. TCS foods not being cold held at the proper temperature (critical repeat, corrected). Observed multiple TCS foods on salad and hibachi buffet held above 41 degrees (watermelon, banana pudding, rice pudding, cut tomatoes/cucumber salad, boiled eggs, sprouts, imitation crab, cooked lo mein noodles, cooked broccoli). PIC voluntarily discarded one pan of all listed foods. Also observed shredded cheese in Pepsi reach-in cooler to have internal temperature of 46 degrees. PIC voluntarily discarded 10 pounds of shredded cheese. Insufficient air gap between the flood rim and the water supply inlet (critical repeat, corrected). At re-inspection, no air gap observed at drain hose for bulk ice machine.

Winchester Terrace Nursing Home, 70 Winchester Road, Mansfield, Aug. 17. Ready-to-eat, TCS food not properly discarded when required (critical repeat, corrected during inspection). Observed a pan of brown gravy in the cooler with a prep date of Aug. 9. PIC voluntarily discarded gravy. Presence of live insects (critical). Observed fruit flies in the dry storage room and near the juice dispenser in the kitchen. Correct by Aug. 24. Manual wash solution not being maintained at 110 degrees or above (critical repeat, corrected). Observed the wash water in the three-compartment sink at 93 degrees. Water source and/or hot water capacity not sufficient (critical). Hot water at the three-compartment sink reached 107 degrees at inspection. Correct by Aug. 24. Insufficient air gap between the flood rim and the water supply inlet (critical). Observed funnels around the drain pipes of the ice machine and food prep sink that were eliminating the air gaps. Correct by Aug. 24.

Wendy's Old Fashioned Hamburgers, 653 N. Lexington Springmill Road, Mansfield, Aug. 18. PIC unable to demonstrate knowledge by having no critical violations (critical). Correct by Aug. 25. Written procedures and plans are not maintained and implemented (critical, corrected during inspection). Observed facility to have time in lieu of temperature policy for multiple foods on make lines. However, facility is not following policy as time stamp was not observed on shredded cheese. TCS foods not being cold held at the proper temperature (critical repeat, corrected). Observed TCS foods in small reach-in cooler near drive-thru to be held above 41 degrees for undetermined amount of time (six bottles of chocolate milk, 22 bottles of white milk, seven packets of Caesar dressing, 14 packets of pomegranate vinaigrette, five packets of ranch dressing and nine packets of champagne vinaigrette). PIC voluntarily discarded listed foods. Also observed pans of bacon sitting on cart with internal temperature between 64-67 degrees. PIC had employee cook bacon. Improper use of time as a public health control - 4 hours (critical, corrected). Continued to observe shredded cheese placed on drive-thru make line without time stamp to indicate when product was pulled from walk-in cooler.

Taco Bell #30674, 1194 Park Avenue West, Mansfield, Aug. 21. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed cinnamon twist uncovered. Quaternary ammonium sanitizing solution at incorrect temperature, concentration and/or water hardness (critical repeat, corrected). Observed quaternary ammonium solution in the warewashing area used to wash dishes below 200 ppm. TCS foods not being hot held at the proper temperature (critical, corrected). Observed rice being hot held in the upright hot holding unit to have an internal temperature of 118-130 degrees. PIC voluntarily discarded 4 cups of rice. TCS foods not being cold held at the proper temperature (critical, corrected). Observed sour cream tube to have an internal temperature of 44-45 degrees. Sour cream was voluntarily discarded.

Oriental Express, 2169 Richland Mall, Mansfield, Aug. 21. PIC unable to demonstrate knowledge by having no critical violations (critical). Correct by Aug. 28. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Observed bowl of raw chicken stored on shelf directly above cooked fried chicken in walk-in cooler. Quaternary ammonium sanitizing solution at incorrect temperature, concentration and/or water hardness (critical repeat). Observed working container of quat sanitizer at greater than 400 ppm. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed cutting surface of blade on can opener to contain visible residue build up. TCS foods not being hot held at the proper temperature (critical, corrected). Observed cooked foods (cooked egg rolls) on hot holding counter held below 135 degrees for undetermined amount of time. PIC voluntarily discarded egg rolls. Observed bowl of steamed vegetables in service window to have internal temperature of 118-120 degrees. PIC had employee reheat steamed vegetables to 200 degrees. TCS foods not being cold held at the proper temperature (critical, corrected). Observed multiple TCS foods in fresh foods reach-in cooler to be held with an internal temperature of above 41 degrees for an undetermined amount of time (wonton wrappers, cut potatoes, butter, eggs, cooked egg rolls, lo mein noodles). PIC discarded listed foods. Food contact surfaces not easily cleanable (critical, corrected). Observed employee use a cut and melted single use plastic vinegar container to scoop cooked vegetables out of wok. PIC removed container from area.

Springmill Corner Market, 939 Springmill St., Mansfield, Aug. 21. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical repeat, corrected during inspection). Observed no employee illness agreements signed for employee. Food not properly protected from contamination by separation, packaging and segregation (critical). Observed raw chicken breast and chicken liver to be stored above sausages, beef and pork in the retail freezer. Correct by re-inspection Aug. 28. Improper storage of poisonous or toxic materials (critical repeat, corrected). Observed soap pads and sanitizer spray bottles stored over the top of the deli freezer.

Lexington Preschool, 890 W. Fourth St., Mansfield, Aug. 21. TCS foods not being cold held at the proper temperature (critical). Cold-holding unit measured at ambient temp of 49 degrees. Please adjust cooler to lower temperatures below 41 degrees.

Wingate By Wyndham, 2069 Walker Lake Road, Ontario, Aug. 22. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed orange juice and container of yogurt in breakfast buffet area to have internal temperatures held above 41 degrees for undetermined amount of time. Employee discarded orange juice and yogurt. Improper storage of poisonous or toxic materials (critical, corrected). Observed can of Raid stored on top of cabinets at three-compartment sink area. PIC removed can from kitchen. Presence of live insects, rodents and other pests (critical repeat). Observed multiple small flying insects in breakfast buffet area and kitchen area, as well as rodent droppings inside cabinets in breakfast buffet area and kitchen area. Correct by Aug. 29.

Walmart #1539, 359 N. Lexington Springmill Road, Mansfield, Aug. 22. Presence of live insects, rodents and other pests (critical repeat). Observed multiple flying insects in various areas throughout facility. At re-inspection, continued to observed flying insects. Correct by Aug. 30.

Chinatown, 283 Ashland Road, Mansfield, Aug. 22. Presence of live insects and/or other pests (critical). Observed flies in back room where food is being prepped. Provide fly control measure, such as blue lights or glue strips. Correct by Sept. 19.
