The following restaurant inspections with critical violations were conducted by Richland Public Health on Aug. 11:

The Golden Burrito Mexican Grill, 574 Park Avenue East, Mansfield, Aug. 11. TCS foods not being cold held at the proper temperature (critical repeat). Observed True reach-in cooler reading 54 degrees. Advised person in charge (PIC) to remove perishable food items. Correct by Aug. 14.

Wishmaker House B & B and Winery, 116 Main St., Bellville, Aug. 11. Handwashing sink is being used for purposes other than handwashing (critical, corrected during inspection). Wiping cloth observed stored at hand wash station. TCS foods not being cold held at the proper temperature (critical, corrected). Heavy cream (dairy) measured at 47 degrees inside Saturn cooler. Improper storage of poisonous or toxic materials (critical, corrected). Sanitizers and cleaning sprays observed stored adjacent to sous vide set up. Improper storage of poisonous or toxic materials (critical, corrected). Glass cleaner spray observed stored beside at food prep table. Improper packaging of food using reduced oxygen packaging without a variance (critical). Sous vide process was being used without required HACCP plan. Correct by Aug. 28.

7-Eleven Store No. 36173 A, 26 W. Main St., Lexington, Aug. 11. TCS foods not being hot held at the proper temperature (critical repeat, corrected during inspection). Observed recently prepared fried chicken in the warming cabinet at 125 degrees. PIC reheated chicken to 190 degrees. TCS foods not being hot held at the proper temperature (critical repeat, corrected). Observed honey chicken at 122 degrees and pizza at 128 degrees in the warming cabinet. PIC voluntarily discarded chicken and pizza. TCS foods not being cold held at the proper temperature (critical, corrected). Observed a gallon of milk with an internal temperature of 47 degrees. PIC voluntarily discarded milk.

Hong Kong Buffet, 331 N. Lexington Springmill Road, Mansfield, Aug. 11. PIC did not ensure employees are properly maintaining temperatures of TCS foods during hot and cold storage (critical repeat). Correct by Aug. 18. PIC unable to demonstrate knowledge by having no critical violations (critical repeat). Correct by Aug. 18. Written procedures and plans are not maintained and implemented (critical repeat). Facility not following time in lieu policy as no time stamps observed. Correct by Aug. 18. Food employee(s) did not wash hands when required (critical, corrected during inspection). Observed employee removed gloves and attempt to return to station. Food package(s) received in poor condition (critical, corrected). Observed can of soy sauce and can of ketchup in stock rotation to contain dents along seals. PIC removed cans from stock to be returned or discarded. Food not properly protected from contamination by separation, packaging and segregation (critical repeat). Observed multiple foods uncovered throughout facility. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected). Observed food employee prepping raw shrimp on prep table directly above ready-to-eat bananas stored on shelf below. Also observed raw eggs stored directly above prepared cake and pot stickers in walk-in cooler. Single-use gloves used improperly (critical, corrected). Observed employee use gloved hands to grab cooked chicken and place in fryer, followed by rinsing single use gloves in prep sink and then attempt to return to station on cook line without removing gloves and washing hands. Equipment food-contact surfaces or utensils are unclean (critical repeat, corrected). Observed vegetable shredder and peeler to contain residue build up. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed vegetable chopper and grinder to contain visible residue build up. Equipment food-contact surfaces or utensils are unclean (critical). Observed food contact surfaces of ice bin storage bin in kitchen are to contain visible residues. Correct immediately. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed the interior food contact portions of the ice dispensers on the fountain beverage machines in the servers area to contain visible pink and black residue build up. PIC removed ice dispensers from service. Equipment food-contact surfaces or utensils are unclean (critical repeat, corrected). Observed multiple lids with visible leftover residue from previously covered foods. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical repeat, corrected). Observed chlorine sanitizer in working sanitizer bucket to be at 0 ppm. Equipment food-contact surfaces or utensils are unclean (critical repeat). Observed visible wet and dry residue on chicken mixer. Correct immediately. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed cutting surface of can opener to container visible residue build up. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed scoops for salt and pepper to contain visible residue build up. Raw animal food not cooked to required time and temperature (critical, corrected). Observed internal temperature of raw chicken just cooked in wok station to have internal temperature of only 153 degrees. TCS foods not being hot held at the proper temperature (critical, corrected). Observed sweet and sour chicken on hot buffet in dining are to have internal temperature of 123 degrees. PIC discarded chicken. TCS foods not being cold held at the proper temperature (critical repeat, corrected). Observed TCS (time\temperature controlled for safety) house-made sauces in buffet in hibachi area held above 41 degrees for undetermined amount of time. Observed food TCS foods on salad buffet bar and hibachi buffet bar held with an internal temperature above 41 degrees for an undetermined amount of time. PIC discarded sauces and listed salad bar and hibachi buffet foods. TCS foods not being cold held at the proper temperature (critical repeat, corrected). Observed multiple TCS foods not properly cold held with an internal temperature of 41 degrees or less in back kitchen area, including six pans of spinach/crab salad, two pans of macaroni & cheese, and 30 pounds of raw shrimp. All listed foods were discarded. Improper use of time as a public health control - 4 hours (critical repeat, corrected). Facility has written policy indicating use of stickers to mark the time sauces are placed in hibachi area. No time stickers were observed. PIC discarded sauces. Presence of live insects (critical). Observed multiple flying insects throughout facility. Correct by Aug. 18. Food contact surfaces not easily cleanable (critical repeat). Observed the interior lining of the rice cooker pot to be peeling/chipping, rendering surface no longer easily cleanable. Correct by Aug. 18. Equipment and/or utensils improper construction (critical repeat, corrected). Observed cardboard box used to store whole potatoes. Food contact surfaces not easily cleanable (critical, corrected). Observed salt and pepper container with areas that were observed to be cut and not easily cleanable. PIC discarded container. Insufficient air gap between the flood rim and the water supply inlet (critical repeat). Observed no air gap from drain hose of bulk ice machine at floor drain. Correct by Aug. 18. No air gap or approved backflow prevention device on the plumbing system (critical repeat, corrected). Observed no backflow on continuous pressure hose. PIC removed continuous pressure hose during inspection.

Wendy's Old Fashioned Hamburgers, 653 N. Lexington Springmill Road, Mansfield, Aug. 11. PIC unable to demonstrate knowledge by having no critical violations (critical repeat). Correct by Aug. 18. Improper use of time as a public health control - 4 hours (critical repeat, corrected during inspection). During re-inspection, observed shredded cheese on make line not marked to indicate time product was pulled from cooler or marked to indicate discard within a 4-hour time period. PIC had employee discard shredded cheese.
