Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between Aug. 14 and Aug. 16:

Meijer #126, 1355 N. Lexington Springmill Road, Ontario, Aug. 14. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected during inspection). Observed packages of ground lamb stored directly above lamb chops and lamb roast in retail area. Also observed packages of ready-to-eat Canadian bacon stored over/directly next to raw pork bacon. TCS foods not being cold held at the proper temperature (critical, corrected). Observed container of chip dip in retail area to have internal temperature of 44 degrees. PIC discarded chip dip. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical repeat, corrected). Observed open packages of cheeses in deli case not on ODA cheese exemption list date marked for longer than 7 days total. Person in charge (PIC) corrected dates on cheeses observed to still be within 7 day holding period and discarded 5 pounds of horseradish cheddar, 2.35 pounds of yellow American and 2.97 pounds of sharp cheddar American.

Athens Greek Restaurant, 41 S. Lexington Springmill Road, Ontario, Aug. 14. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed a gallon of milk at 46 degrees in the reach-in cooler by the walk-in freezer. PIC voluntarily discarded milk. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed an opened package of hot dogs without a date mark.

Mansfield Restaurant Inc./Westside, 1090 Park Avenue West, Mansfield, Aug. 14. Person in charge not ensuring that employees verify foods are approved and received properly when delivered during non-operating hours (critical, corrected during inspection). PIC stated that no temperatures are taken on products received on trucks. Education was provided. PIC unable to demonstrate knowledge by having no critical violations (critical). Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected). Observed raw hamburger meat sitting over hash browns in the upright cooler. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed make-line cutting board/prep surface being dripped on by ventilation condensate. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected). Observed 0 ppm concentration of quaternary ammonium solution in the buckets at the server station. PIC had remade to proper 200-400 ppm concentration. TCS foods not being cold held at the proper temperature (critical repeat, corrected). Observed various TCS products on the make line in the cooler to have internal temperatures of 51-55 degrees. PIC voluntarily discarded products. TCS foods not being cold held at the proper temperature (critical, corrected). Observed several TCS pies in the pie cooler with internal temperature of 45-49 degrees. PIC voluntarily discarded out of temperature product. Improper storage of poisonous or toxic materials (critical, corrected). Observed restroom deodorant can sitting over condiments on a shelf. Presence of live insects (critical). Observed black flying insects throughout the facility. Correct by re-inspection Aug. 21.

The Whiffletree Restaurant/Patricia's Home Town Dining LLP, 28 Main St., Butler, Aug. 15. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Cheese broccoli soup not in use was observed without a lid.

Tito's Cantina and Grill, 320½ E. Main St., Lexington, Aug. 15. PIC unable to demonstrate knowledge of cleaning and sanitizing (critical, corrected during inspection). PIC stated that utensils used for TCS foods not held with temperature are cleaned at night. Ensure utensils are cleaned at a four hour frequency. PIC unable to demonstrate knowledge of cleaning and sanitizing (critical, corrected). PIC was unable to state sanitizer strengths for bleach. Ensure sanitizer strength for bleach is between 50-100 ppm. PIC unable to demonstrate knowledge by having no critical violations (critical). Equipment food-contact surfaces or utensils are unclean (critical repeat, corrected). Observed the food contact portion of the following pieces of equipment to have visible residue: can opener blade, potato slicer and two knife blades hung in the clean storage area. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed house-made cilantro ranch to have no date mark. Improper storage of poisonous or toxic materials (critical repeat, corrected). Observed cans of fuel and chafing fuel containers to be stored above single use articles. Improper use of a poisonous or toxic substance (critical, corrected). Observed Hot Shot chemical hanging pest pads to be hung in the dry storage area .Ensure all pest control/chemical is being applied by a licensed pest control operator and is safe for use in a kitchen. PIC removed at time of inspection. Presence of live insects (critical). Observed black flying insects throughout the facility and small grey flying insects in the dry storage area. Correct by re-inspection Aug. 22. Food contact surfaces not easily cleanable (critical, corrected). Observed four knives with chips missing from blade, rendering the surfaces no longer smooth, durable and easily cleanable. PIC discontinued use of knives. No air gap or approved backflow prevention device on the plumbing system (critical). Correct by re-inspection Aug. 22.

Tall Timber Inn/Tri Valley DBA, 37 Main St., Butler, Aug. 15. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Milk observed stored in cooler since Sunday without proper date marking.

Walmart #1539, 359 N. Lexington Springmill Road, Mansfield, Aug. 15. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed TCS foods in produce end cap cooler held above 41 degrees for an undetermined amount of time: three Philly cheese calzones; one chicken pot pie; two Marketside breakfast pizzas; one Marketside five cheese pizza; and one Marketside pepperoni pizza. PIC voluntarily discarded listed foods. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical repeat, corrected). Observed pepper jack cheese date marked incorrectly for 21 days. PIC corrected discard date on sticker to indicate consumption or discard within 7 days total. Observed packages of discounted meat and cheese to display the date the meat\cheese was sliced and not the actual date opened. PIC did pull packages from retail cooler to properly record date. Presence of live insects, rodents and other pests (critical). Observed multiple flying insects in various areas throughout facility (bakery, deli, mop sink near grocery receiving area). Correct by Aug. 22.

Five Guys Burgers & Fries, 994 N. Lexington-Springmill Road 160, Ontario, Aug. 16. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected during inspection). Observed uncovered cooked bacon slices on the make line.

Outback Steakhouse, 820 Lexington Springmill Road, Mansfield, Aug. 16. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed shredded lettuce, shredded cheese and sliced tomatoes in cold-holding coffin table cooler to be held above 41 degrees for undetermined amount of time. PIC voluntarily discarded listed foods. Ready-to-eat, TCS food not properly discarded when required (critical, corrected). Observed house-made ranch dressing and house-made honey mustard not discarded by required date. PIC discarded both sauces. Presence of live insects (critical). Observed multiple large and small flying insects in back prep area, bar area and in utility area. Correct by Aug. 23.
