Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between May 5 and May 10:

TownePlace Suites, 1125 Guest Court, Ontario, May 5. TCS foods not being cold held at the proper temperature (critical repeat, corrected during inspection). Observed oat milk in glass carafe in self-service breakfast area to have internal temperature of 53 degrees as well as individual portioned cream cheese and butter to have internal temperatures above 41 degrees during inspection. Person in charge (PIC) discarded oat milk, three individual packets of cream cheese and four individual packets of butter.

Friends Convenient Mart, 1605 Fourth St., Ontario, May 5. Handwashing sink not accessible (critical, corrected during inspection). Observed travel coffee mug in handwashing sink. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed open pack of sliced American cheese sitting out on retail counter to have internal temperature of 50 degrees. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical repeat, corrected during inspection). Observed open bags of bologna date marked for June 1, hot dogs date marked for April 26, and American cheese date marked June 1. PIC verified all listed foods were delivered/opened May 1 and correctly date marked items for May 1. Missing and/or incomplete allergen declaration(s) (critical, corrected during inspection). Observed peanut butter cookies and chocolate chunk cookies made and packaged in the facility, offered for customer self service, missing allergen information. Employee removed cookies and placed behind service counter.

Subway #30929, 1521 Lexington Ave., Mansfield, May 5. Improper storage of poisonous or toxic materials (critical, corrected during inspection). Bathroom cleaner observed above microwave and clean gloves. Food contact surfaces not easily cleanable (critical, corrected during inspection). Scissors that were not able to be separated used to open food bags. PIC removed from service. Manual wash solution not being maintained at 110 degrees or above (critical, corrected during inspection). Wash solution was 84 degrees.

McDonald's - Hanley Road #5500, 25 W. Hanley Road, Mansfield, May 5. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical). Observed chemical sanitizing machine to be 0 ppm. Correct by re-inspection May 12. Insufficient air gap between the flood rim and the water supply inlet (critical, corrected during inspection). Observed no air gap under the bag in bag rack and under the lobby soda machine.

Fair Board Auxiliary - Fairhaven Hall, 750 N. Home Road, Mansfield, May 6. Food not properly protected from contamination by separation, packaging and segregation. (critical, corrected during inspection). Observed a pan of raw bacon on a shelf above bagged lettuce inside the reach-in cooler. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed a package of ready-to-eat hot dogs under a package of raw ground beef in the walk-in cooler.

Fair Board Auxiliary - Youth Hall Cafeteria, 750 N. Home Road, Mansfield, May 6. Food not properly protected from contamination by separation, packaging, and segregation (critical repeat, corrected during inspection). Observed a package of ready-to-eat bologna under a package of raw ground beef in the True reach-in cooler. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed the top layer of bologna and ham at 45 degrees in the top of the make table cooler. PIC voluntarily discarded bologna and ham.

Park Avenue Child Care And Education, 812 Park Avenue East, Mansfield, May 8. No air gap or approved backflow prevention device on the plumbing system (critical, corrected during inspection). Observed no air gap on vegetable wash sink. Plumbing code required air gap to be two times the diameter of the pipe size.

Shell Gas Station, 77 S. Diamond St., Mansfield, May 9. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical). Observed quat solution located at three-compartment sink to be at 100 ppm. Correct by May 12.

McDonald's - Richland Mall #3350, 666 Lexington Springmill Road, Mansfield, May 9. Food contact surfaces not easily cleanable (critical, corrected during inspection). Observed scissors stored in automatic creamer dispensers used to cut open food packaging not easily cleanable. PIC removed scissors from use.

Brown Derby Roadhouse, 3985 Park Avenue West, Ontario, May 9. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed four raw beef steaks sitting on a rack with an internal temperature 62 degrees. Ensure when the cutting process happens the meat maintains a temperature of 41 degrees or below at all times. PIC voluntary discarded three-and-a-half pounds of beef steaks.

Taco Bell #15366, 996 Ashland Road, Mansfield, May 9. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical). Observed 0 ppm quat sanitizer at three-compartment sink. Correct by May 12. Equipment and/or utensils improper construction (critical). Observed the hardened rubber on paddle-style stirrers has discolored and deformed. Replace all paddles by May 12.

Tacos & Margaritas, 288 Lexington Ave., Mansfield, May 10. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed can opener to have visible residue on the blade. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Observed whipped cream that PIC verified was opened on May 5. Also observed cheese in the walk-in cooler with the wrong date, PIC was able to verify open date. Missing and/or incomplete allergen declaration(s) (critical repeat, corrected during inspection). Observed spoon candies manufactured and packaged off site without proper labeling for allergens. PIC was unable to produce original packaging and removed candies from sale until proper labeling can be obtained.

Clear Fork High School, 987 Ohio 97, Bellville, May 10. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Fully-cooked ham that had been open more than 24 hours was not date marked. Improper storage of poisonous or toxic materials (critical, corrected during inspection). Deodorizing spray observed stored next to single use gloves.

Joe & Mary's Market, Adm. Sturges Inc., 162 Sturges Ave., Mansfield, May 10. Presence of rodents (critical repeat). Observed rodent droppings behind the mop sink in the storage area and behind shelving and ice cream freezer near the walk-in dairy cooler door. At re-inspection observed no change. Correct by May 17.
