Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between Sept. 15 and Sept. 19:

Otterbacher Food Service, 20 High St., Shiloh, Sept. 15. Handwashing sink not accessible (critical, corrected during inspection). Observed a box of potatoes setting on the hand sink.

IHOP, 2190 Walker Lake Road, Ontario, Sept. 15. Food employee(s) did not wash hands when required (critical, corrected during inspection). Employee on cook line observed donning new gloves without first washing hands. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Chicken tenders in reach-in Traulsen freezer on cook line was observed subject to splash, dirt and dust. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Home fries in Traulsen reach-in freezer on cook line observed uncovered and subject to sneeze, splash, dirt and dust. TCS foods not being cold held at the proper temperature (critical, corrected). Tomatoes in reach-in cooler on cook line measured at 51 degrees. Listed item voluntarily discarded. Ready-to-eat, TCS food not properly discarded when required (critical, corrected). Sliced and diced tomatoes observed in walk-in cooler even though they were datemarked to be disposed of Sept. 14. Denvier mix was observed in walk-in but was scheduled to be discarded Sept. 8. Listed items were voluntarily discarded. Food contact surfaces not easily cleanable (critical, corrected). Spatula observed above three-compartment sink that was cracked, no longer smooth and cleanable. Listed item was voluntarily discarded.

Mansfield First EPC Preschool, 399 S. Trimble Road, Mansfield, Sept. 16. TCS foods not being cold held at the proper temperature (critical, corrected). Observed TCS foods for preschool students (milk, cream cheese, syrup, yogurt, mozzarella cheese sticks) in refrigerator to have internal temperatures of between 45 and 48 degrees. Person in charge (PIC) dumped milk and discarded all listed foods during inspection. Ensure TCS food are held at or below 41 degrees for safety.

McDonald's - Hanley Rd #5500, 25 W. Hanley Road, Mansfield, Sept. 16. Presence of live insects (critical repeat). Small flying insects observed under cabinet by drain across from registers. Please have removed by re-inspection.

Chipotle Mexican Grill #2207, 2470 O'Possum Run Road, Mansfield, Sept. 16. Food not properly protected from contamination by separation, packaging, and segregation (critical repeat, corrected during inspection). Observed salt and cilantro on rice table that is not covered and not in use.

500 Park Avenue Café, 500 Park Avenue West, Mansfield, Sept. 16. Ready-to-eat, TCS food not properly discarded when required (critical, corrected during inspection). Peppers observed in freezer were old and past time of use. Listed food voluntarily removed and discarded.

Jersey Mike's Subs, 783 N. Lexington Springmill Road, Ontario, Sept. 19. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected during inspection). Observed bacon stored in container along front make line to not be covered. TCS foods not being cold held at the proper temperature (critical, corrected). Multiple TCS foods observed between 43-45 degrees in deli display case. PIC placed unopened meat with internal temperature of 43 degrees in walk-in cooler with ice bath to be rapidly cooled. PIC placed opened meats that had just been placed in display case prior to opening back into walk-in cooler as well. Deli meats with internal temperature of 44-45 degrees were discarded. TCS sauces in bottom of deli cooler observed at 44 degrees. PIC discard sauces at time of inspection.

The Local @ 97, 58 Ohio 97, Lexington, Sept. 19. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Cooking bowl of flour used for frying pickles observed stored on top shelf in cook line subject to splash, dirt and dust. Flour voluntarily discarded. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Ready-to-eat onions observed stored in walk-in cooler on shelf with no covering. Food equipment surfaces not cleaned at required frequency (critical, corrected). Can opener near pizza oven observed dirty with food residue. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Several knives observed stored on cook line with food residue. Improper cooling of TCS food (critical). PIC showed cooling records where cooling process was started at 165 degrees Fahrenheit instead of required 135 degrees. Correct by Oct. 5. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Cooked chicken breasts prepared Sept. 17 observed stored in walk-in cooler without proper datemarking. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Marinara sauce in walk-in cooler observed stored over 24 hours without datemarking. PIC voluntarily discarded. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Garlic mashed potatoes observed stored in walk-in cooler for over 24 hours without proper datemarking. PIC voluntarily discarded. Presence of live insects, rodents, and other pests (critical). Numerous flying insects observed throughout facility. Correct by Oct. 5.

Campus Café, 1640 University Drive, Mansfield, Sept. 19. PIC unable to demonstrate knowledge by having no critical violations (critical). PIC not ensuring employees are properly sanitizing equipment and utensils and monitoring of sanitizer parameters (critical, corrected during inspection). Employees are not sanitizing temperature probe before taking temperatures. PIC brought out alcohol wipes for temperature probes. Food employee(s) did not wash hands when required (critical, corrected). Observed food employee not washing hands before putting on gloves. TCS foods not being cold held at the proper temperature (critical, corrected). Approximately two pounds of cut lettuce was voluntarily destroyed due to the fact the temperature was 45 degrees internally on the salad bar. Presence of live rodents (critical). Observed mouse droppings in pop storage area. Remove all evidence of droppings, clean area and monitor for further activity. Re-inspection Sept. 26.

Roosters Mansfield, 2140 W. 4th St., Mansfield, Sept. 19. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed residue on the can opener blade. Ready-to-eat, TCS food not properly discarded when required (critical, corrected). Observed a container of burrito chicken in the make table cooler with a prep date of Sept. 11 and a use by date of Sept. 17. PIC voluntarily discarded chicken.
