Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between Oct. 12 and Oct. 17:

Mickey Mart #32, 61 Mill Road, Bellville, Oct. 12. Person in charge (PIC) unable to demonstrate knowledge of cleaning and sanitizing (critical repeat, corrected during inspection). PIC was unable to demonstrate record of regular cleaning and sanitizing of F'Real cold holding equipment. Proper instruction provided to PIC for accessing automated record of cleaning. Improper display of poisonous or toxic materials offered for retail sale (critical, corrected). Snickers candy bars observed stored beside chemical spray cleaners.

Crestview Middle School, 1575 Ohio 96 E., Ashland, Oct. 12. TCS foods not being hot held at the proper temperature (critical, corrected during inspection). Potato wedges on serve line measured at only 124 degrees. Listed items removed from serve line and reheated to 182 degrees.

Bob Evans Restaurants LLC #485, 2100 W. Fourth St., Mansfield, Oct. 12. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected during inspection). Observed frozen veggies and French fries in reach-in McCall freezer next to fryer uncovered/unprotected from possible contamination. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed metal ladle and metal tongs in clean utensil storage to have visible dried residue on food contact portion of utensil. Ready-to-eat, TCS food not properly discarded when required (critical, corrected). Observed container of cole slaw in walk-in cooler held past the observed discard date of Oct. 3, as well as a container of sliced turkey in reach-in McCall cooler next to fryer area held past the observed discard date of Oct. 9. PIC discarded listed foods.

Bob Evans Restaurants LLC #62, 525 N. Trimble Road, Mansfield, Oct. 13. Improper use of time as a public health control - 4 hours (critical, corrected during inspection). Observed a container of liquid egg mix on the shelf below the grill without a time sticker. PIC voluntarily discarded egg mix and placed a time sticker on a new container of egg mix.

Countryside Pizza, 2256 Park Avenue East, Mansfield, Oct. 16. Insufficient air gap between the flood rim and the water supply inlet (critical). Observed air-gap above lower catch bowl and pipe.Air gap is required to prevent back siphonage of gray water. Correct by Nov. 16.

7-Eleven Store No. 36173 A, 26 W. Main St., Lexington, Oct. 16. Handwashing sink is being used for purposes other than handwashing (critical, corrected during inspection). Observed aluminum cans in the back hand-washing sink by the three-compartment sink. TCS foods not being cold held at the proper temperature (critical repeat, corrected). Internal temperature of Kraft cheese (46 degrees) and Sargento cheese (53 degrees) in the open-air cooler. PIC voluntarily discarded approximately 10 packages of cheese. TCS foods not being cold held at the proper temperature (critical, corrected). Observed French vanilla and mocha flavored latte mixes at 50 degrees in the dispenser. PIC voluntarily discarded latte mixes.

Ontario Duchess #1249, 3675 Park Avenue West, Ontario, Oct. 16. TCS foods not being cold held at the proper temperature (critical repeat, corrected during inspection). Observed multiple TCS foods in lower portion of grab-in-go cooler to have internal temperatures above 41 degrees for an undetermined amount of time. PIC discarded TCS foods above 41 degrees.

Ohio Food Mart LLC, 382 Lexington Ave., Mansfield, Oct. 16. Person in charge did not ensure that employees observed foods as they were received (critical, corrected during inspection). PIC stated they do not take temperatures upon receipt of cold-held TCS products. Ensure deliveries are being observed to ensure product is being delivered at appropriate temperatures of 41 or below. PIC unable to demonstrate knowledge by having no critical violations (critical repeat, corrected). All critical violations corrected at time of inspection. Handwashing sink is being used for purposes other than handwashing (critical repeat, corrected). Observed both handwashing sinks being used for storing dishes or pop/beer containers. Handwashing sink not accessible (critical repeat, corrected). Observed handwashing sink in the storage area to be blocked by a liquor storage unit.

Main Street Pit Stop, 56 W. Main St., Lexington, Oct. 16. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed raw bacon on a shelf above ready-to-eat products inside the walk-in cooler.

Keat's Coffee Ltd., 335 Glessner Ave., Mansfield, Oct. 17. Food contact surfaces not easily cleanable (critical, corrected during inspection). Observed scissors being used for cutting opening food packages that are not smooth, durable, non-absorbent or easily cleanable. PIC discontinued use of scissors.

Marco's Pizza #1085, 1305 Lexington Ave., Mansfield, Oct. 17. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed sliced ham in the top portion of the make table cooler at 50 degrees. PIC voluntarily discarded ham.

Applebee's Neighborhood Grill & Bar, 1023 N. Lexington Springmill Road, Mansfield, Oct. 17. TCS foods not being cold held at the proper temperature (critical repeat, corrected during inspection). Observed parmesan sauce on make line cooler to have internal temperature of 48 degrees. PIC discarded sauce. Observed TCS drinks and mixes (milk, sweet & sour mix, blue mix, cranberry juice) in Traulsen cooler in bar area to have internal temperature of 43 degrees consistently. PIC relocated listed drinks and mixes to another cooler in bar area observed to have ambient temperature of 38 degrees.
