Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between Oct. 2 and Oct. 5:

Deer Ridge Golf Acquistion LLC, 900 Comfort Drive, Bellville, Oct. 2. Insufficient air gap between the flood rim and the water supply inlet (critical repeat). Observed drain from ice storage bin and ice maker in catering kitchen area on lower level without sufficient air gap between flood rim level of floor drain and end of drain hose. At re-inspection, owner and person in charge (PIC) state they have been in contact with the plumber to get the work scheduled. Discussed contacting specialist at Richland Public Health once repairs have been made to schedule follow up.

Hissho Sushi @ Meijer #126, 1355 N. Lexington-Springmill Rd., Mansfield, Oct. 3. PIC unable to demonstrate knowledge of HACCP/Hazard Analysis Critical Control Point (critical, corrected during inspection). PIC stated the ratio of water to rice used for the rice slurry is 1 cup rice to 3 cups distilled water. Discussed that per the Ohio uniform food code and per facilities HACCP plan, the ratio for the rice slurry is one-fourth rice (2 ounces) to three-fourths distilled water (6 ounces). Observed HACCP plan to indicate this during inspection and PIC used correct ratio when testing the pH of the acidified rice.

Pulp Juice and Smoothie Bar, 994 Lexington Springmill Road, Ontario, Oct. 3. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Observed an open bag of shredded mozzarella/provolone blend cheese not date marked. PIC unable to verify when bag of cheese was opened and discarded cheese. Cheese date marking exemption list provided during inspection and discussed ensuring TCS foods held longer than 24 hours are date marked to indicate day or prep\open or date of discard.

OhioHealth Mansfield Hospital, 335 Glessner Ave., Mansfield, Oct. 3. PIC unable to demonstrate knowledge by having no critical violations (critical repeat, corrected during inspection). Presence of live insects (critical repeat, corrected during inspection). Observed small flying insects behind unused sink in the room service cook area, under the rethermilizer/boilers, behind digester in the warewashing area, near the chemical rack in the warewashing area, and behind the conveyor warewashing machines. At reinspection observed significant decrease in small flying insects; some still present. PIC provided pest control summaries from Sept. 27 and Oct. 2. Facility is to keep working with pest control operator to get the pests under control and to send monthly service summaries for the next three months.

Panera Bread #4780, 2070 Walker Lake Road, Mansfield, Oct. 3. Insufficient air gap between the flood rim and the water supply inlet (critical, corrected during inspection). Observed plastic cup added to flow drain and eliminating air gap for drain from ice maker. PIC removed plastic cup during inspection.

Everbowl, 2074 Walker Lake, Mansfield, Oct. 4. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical repeat, corrected during inspection). Observed working buckets of quaternary ammonium to be at 150 ppm. Employee discarded sanitizer during inspection. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Observed open containers of almond milk and coconut milk in reach-in cooler not date marked. Employee unable to verify when containers were opened and discarded during inspection. Ensure TCS foods held longer than 24 hours are date marked to indicate day of open/prep or date of discard.

Mansfield Christian Elementary School, 500 Logan Road, Mansfield, Oct. 4. PIC did not ensure employees are properly maintaining temperatures of TCS foods during hot and cold storage (critical, corrected during inspection). Hamburgers on counter were measured at 94 degrees. Proper instruction was given. TCS foods not being hot held at the proper temperature (critical, corrected during inspection). Hot holding hamburger measured at 94 degrees. PIC discarded five hamburgers.

The Happy Grape, 300 E. Main St., Lexington, Oct. 4. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected during inspection). Observed a bucket of sanitizer in the bar that was below 200 ppm. PIC refilled solution to a concentration above 200 ppm at the time of inspection.

Butler Clear Fork Adult Center, 20 Henry St., Butler, Oct. 5. Improper washing of hands and arms (critical, corrected during inspection). Food worker observed touching contaminated surface with clean hands. Proper instruction given to PIC.
