Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between May 12 and May 18:

Quick-Mart/Amita Inc. DBA, 563 S. Diamond St., Mansfield, May 12. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Milk shall be properly dated and removed prior to sell by date. Milk was discarded during inspection.

7-Eleven Store # 36178 A, 2424 Possum Run Road, Mansfield, May 13. Handwashing sink not accessible (critical, corrected during inspection). Observed rear hand sink to be inaccessible. Boxes stored in front of sink.

Temple Christian School, 752 N. Stewart Road, Mansfield, May 13. PIC did not ensure employees are proplerly maintaining temperatures of TCS foods during hot and cold storage (critical, corrected during inspection). Lettuce and chicken were not observed in compliance. Food employee(s) did not wash hands when required (critical, corrected during inspection). Worker observed donning new gloves without first washing hands. Single-use gloves used improperly (critical, corrected during inspection). Worker with gloved hands observed touching microwave handles and soiled countertops and then touching chicken and lettuce. Worker obtained clean tongs to prepare food to serve. TCS foods not being hot held at the proper temperature (critical, corrected during inspection). Cooked chicken measured at only 125 degrees. Worker reheated to over 165 degrees. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). TCS lettuce was measured at 47 degrees during food prep. Listed item was put in shallow pan with ice and cooled to 37 degrees.

Crystal Care Center of Mansfield, 1159 Wyandotte Ave., Mansfield, May 13. Insufficient air gap between the flood rim and the water supply inlet (critical). Observed the ice machine drain pipe going into the floor drain and air gap was not visible. Correct by May 20.

Shell #1357, 1357 Lexington Ave., Mansfield, May 16. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical repeat, corrected during inspection). Employee present did not have an employee illness agreement where they had been informed in a verifiable manner. Handwashing sink not accessible (critical, corrected during inspection). Paper towels observed in hand sink next to three compartment sink. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Raw shell eggs observed on shelf above ready-to-eat foods in walk-in cooler. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Small fridge had ambient temperature of 74 degrees. PIC voluntarily discarded two,10 ounce packages of cheddar cheese.

Hong Kong Buffet, 331 N. Lexington Springmill Road 125, Mansfield, May 16. TCS foods not being cold held at the proper temperature (critical repeat, corrected during inspection). Observed multiple products on salad buffet cooler above 41 degrees: broccoli salad - 55 degrees; tomato salad - 45 degrees; tapioca pudding - 49 degrees; banana pudding - 50 degrees; apple salad - 55 degrees. Person in charge (PIC) states all products were prepared that morning from room temperature ingredients. PIC placed products in walk-in cooler to bring down to 41 degrees.

Friends Convenient Mart, 1605 Fourth St., Ontario, May 16. Presence of live rodents (critical repeat). Observed mouse droppings in the back storage area. Remove all evidence of mouse droppings and monitor area for further activity. PIC stated a pest control employee had been in the week before and reported no activity. PIC will call the pest control company to do a new inspection. Correct by May 23. Water source and/or hot water capacity not sufficient (critical repeat). There was only 77 degree water at the three compartment sink, kitchen hand sink, men's room hand sink, and 88 degrees at the women's hand sink. Inspection showed a small hot water heater not sufficient to have hot water at peak usage. Contact licensed plumber and correct by May 23.

Ontario High School, 467 Shelby Ontario Road, Ontario, May 16. PIC did not ensure employees are properly maintaining temperatures of TCS foods during hot and cold storage (critical). Tossed salad, hamburgers and chicken sandwiches on serving line were not cold held, and hot held at proper temperatures during serving time. Listed food items were either discarded or reheated to above 165 degrees.

Ontario Stingel Elementary, 416 Shelby Ontario Road, Ontario, May 16. Person in charge not ensuring proper cooling (critical). PIC stated she does not take temperatures nor monitor time for leftover foods that are cooling. Educational handout and cooling chart provided to PIC. Please complete chart for leftover food to prevent potential contamination from microbial growth that may be pathogenic due to high moisture content from refrigerating warm foods. Correct by May 20. Improper washing of hands and arms (critical, corrected during inspection). Employee observed touching dirty faucet handle with clean hands in process of washing. Improper cooling of TCS food (critical). PIC stated she is not taking temperatures nor monitoring times when warm leftover food is placed in walk-in cooler. To avoid moistening food and allowing environment for potentially pathogenic microorganisms during cooling process, please cool for maximum of two hours for leftover TCS food from 135 to 70 degrees and from 70 to 41 degrees within four hours. Please record on provided chart. Correct by May 20.

Crossroads Church, 1188 Park Avenue West, Mansfield, May 16. Water source and/or hot water capacity not sufficient (critical, corrected during inspection). Water temperature at three compartment sink was 104 degrees. PIC adjusted hot water so temperature was above 110 degrees.

Taco Bell # 35253, 1423 Lexington Ave., Mansfield, May 17. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical repeat, corrected during inspection). Employee present and preparing food had not been informed in a verifiable manner of their responsibility to report. Handwashing sink not accessible (critical, corrected during inspection). Hand sink by ICEE machine had glove in it. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Shears on dining room make line had visible dried on food residue.

Bob Evans Restaurants LLC #485, 2100 W. 4th St., Mansfield, May 17. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected during inspection). Observed working bucket of sanitizer at front counter area to be at 50 ppm. Sanitizing solution was discarded and remade. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed multiple food items between 45 - 54 degrees. PIC voluntarily discarded food. Improper storage of poisonous or toxic materials (critical, corrected during inspection). Observed working container of degreaser amongst clean dishes on shelf at mechanical warewashing unit. PIC placed bottle in chemical storage room.

Storyside Café, 541 Ohio 97 W., Bellville, May 12. Presence of rodents (critical repeat). Observed mouse droppings inside cabinets along back wall. Remove all evidence of mouse droppings, monitor area for further activity, and contact licensed pest control company. Correct by May 18.

Storyside Café, 541 Ohio 97 W., Bellville, May 18. Presence of rodents (critical repeat). Observed continued evidence of mouse activity at re-inspection. Please continue to thoroughly clean and monitor for continued activity. Correct by June 1.
