Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between Oct. 14 and Oct. 20:

Schwebel Baking Company, 2139 Stumbo Road, Ontario, Oct. 14. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). Observed no employee illness agreements on site. Person in charge (PIC) did sign agreements and discussed manner to have others.

Locust Corners Bulk Food & Deli LLC, 6917 Ankneytown Road, Bellville, Oct. 14. No safe handling instructions on packaged raw meat (critical, corrected during inspection). Observed packages of raw pepper bacon in the freezer without safe handling instructions. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected). Observed raw pork bacon on a shelf above ready-to-eat smokie snack sticks in the retail cooler. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical repeat, corrected). Bucket of bleach water at 0 ppm. TCS foods not being cold held at the proper temperature (critical, corrected). Observed a bag of miscellaneous cheese ends in the deli cooler at 50 degrees. PIC voluntarily discarded cheese. Working containers of poisonous or toxic materials not properly labeled (critical, corrected). Observed a bucket of bleach water that was not labeled.

Texas Roadhouse, 515 N. Lexington Springmill Road, Ontario, Oct. 17. Presence of live insects (critical repeat). Observed gnats near syrups in bar area. PIC states he has been in contact with Ecolab to do an assessment of the area to determine problem areas. Facility does have some measures in place already to treat them. Correct by Oct. 31.

Dollar Tree #06925, 600 Springmill St., Mansfield, Oct. 17. Food package(s) received in poor condition (critical, corrected during inspection). Observed four cans of food items dented; observed six cans to be rusted; observed, one bottle and five cans to have labels peeling off. Dented and rusted cans were voluntarily discarded. Items with peeling labels are to be taped back on.

Ohio Food Mart LLC, 382 Lexington Ave., Mansfield, Oct. 17. PIC unable to demonstrate knowledge by having no critical violations (critical). Observed four critical violations. Correct by re-inspection Oct. 20. Handwashing sink not accessible (critical repeat, corrected during inspection). Observed boxes blocking access to the front hand sink. Handwashing sink is being used for purposes other than handwashing (critical repeat). Observed bottles and beer in the storage hand sink. Removed by re-inspection Oct. 20. Handwashing sink is being used for purposes other than handwashing (critical repeat). Observed toothpaste on the front hand sink. Remove by re-inspection Oct. 20. Improper display of poisonous or toxic materials offered for retail sale (critical, corrected). Observed a bottle of Comet bleach cleaner over dog food.

Conard Cafe And Campus Store, 1020 Trimble Road, Mansfield, Oct. 17. Ready-to-eat, TCS food not properly discarded when required (critical, corrected during inspection). Observed pesto mayo sauce prepared in the facility with a prep date of Oct. 7. PIC voluntarily discarded sauce.

Lexington Senior Civic Center, 67 E. Main St., Lexington, Oct. 18. Improper backflow prevention device installed on a water supply system (critical). No backflow prevention device observed on the mop sink faucet. Correct by Oct. 25.

Kentucky Fried Chicken, 301 Ashland Road, Mansfield, Oct. 18. Manual wash solution not being maintained at 110 degrees or above (critical repeat, corrected during inspection). Observed 100 degrees at three-compartment wash station.

St. Mary's Catholic School, 1630 Ashland Road, Mansfield, Oct. 18. PIC did not ensure employees are properly maintaining temperatures of TCS foods during hot and cold storage (critical). PIC did not take temperatures of ham sandwiches prior to putting them on the serving line. Food equipment surfaces not cleaned at required frequency (critical, corrected during inspection). Probe thermometer was observed used without first properly sanitizing thermometer. TCS foods not being cold held at the proper temperature (critical, corrected). TCS ham and cheese sandwiches measured at 56 degrees while on the serving line. Food contact surfaces not easily cleanable (critical, corrected). Clean spatula in drawer observed with uneven edges that were not smooth and easily cleanable. Item voluntarily discarded.

Sam's Club #6407, 1070 N. Lexington Springmill Road, Mansfield, Oct. 18. Hot water for mechanical sanitization is below required temperature (critical repeat). Observed water temperature on dishwasher to be 104 degrees. PIC states he will contact EcoLab for service.

Hong Kong Buffet, 331 N. Lexington Springmill Road 125, Mansfield, Oct. 19. PIC unable to demonstrate knowledge by having no critical violations (critical repeat, corrected during inspection). Observed sauces on hibachi line not properly cold held at 41 degrees or less. PIC corrected temperature violation. TCS foods not being cold held at the proper temperature (critical repeat, corrected). Observed sauces on hibachi station not properly cold held. PIC voluntarily discarded products. TCS foods not being cold held at the proper temperature (critical, corrected). Observed a bowl of raw shrimp thawing on the food prep counter without temperature control. Shrimp was observed to have an internal temperature of 52 degrees. PIC voluntarily discarded product.

Wendy's Old Fashioned Hamburgers, 970 Ohio 97, Lexington, Oct. 19. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed containers of bacon in not covered and protected from contamination. TCS foods not being hot held at the proper temperature (critical, corrected). Observed internal temperature of crispy chicken in hot holding tray along drive-thru make line to be at 129 degrees. PIC had employee discard chicken (five filets). CS foods not being cold held at the proper temperature (critical, corrected). Observed multiple TCS foods in front kitchen area to be above 41 degrees. PIC voluntarily discarded cut romaine lettuce, packets of butter spread and sour cream, and three bottles of fruit purees for lemonade. Manual wash solution not being maintained at 110 degrees or above (critical, corrected). Observed wash water in three-compartment sink to have temperature of 106 degrees.

The Waterford At Mansfield, 1296 S. Trimble Road, Mansfield, Oct. 20. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). PIC unable to locate signed copies of employee illness reporting agreements. Provided RPH's employee illness reporting agreement form during inspection. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed residue inside the nozzle of the juice wand. Ready-to-eat, TCS food not properly discarded when required (critical repeat, corrected). Observed a bag of turkey lunch meat with a slice date of Oct. 8 in the make table cooler. PIC voluntarily discarded turkey at the time of inspection.
