Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between Oct. 19 and Oct. 24:

Rama Tika Retails LLC DBA Friends Food Mart, 1368 N. Main St., Mansfield, Oct. 19. Person in charge (PIC) did not ensure employees are properly maintaining temperatures of TCS foods during hot and cold storage (critical, corrected during inspection). Three pounds of ground beef was left on counter outside of cold holding measuring 57 degrees. Listed item was voluntarily discarded. PIC unable to demonstrate knowledge by having no critical violations (critical). Multiple critical violations observed. Person in charge did not ensure that employees observed foods as they were received (critical). PIC stated workers were not taking temperatures of cold foods upon receipt during deliveries. Person in charge did not ensure employees were effectively cleaning their hands as required (critical, corrected). Worker did not wash hands prior to donning new gloves. PIC not ensuring employees are properly sanitizing equipment and utensils and monitoring of sanitizer parameters (critical). PIC stated fryer baskets were washed and sanitized every 48 hours instead of every 24 hours. Food employee(s) did not wash hands when required (critical, corrected). Worker observed donning new gloves without first washing hands. Food package(s) received in poor condition (critical, corrected). Two cans of vanilla pudding, one can of cheddar sauce observed damaged in dry goods storage. Listed items were voluntarily discarded. Food not properly protected from contamination by separation, packaging and segregation (critical). Container of sugar on food prep table was observed uncovered and subject to sneeze, splash, dirt and dust. Several chemical cleaner spray bottles observed stored on shelf above food prep table. Self-service operation not properly monitored (critical, corrected). Two lids of frozen Minute Maid beverage machines observed partially off with beverage exposed in consumer self-service area. Food display not properly protected from contamination by consumers (critical). Ready-to-eat apples observed displayed near hot hold display cabinet that were unwrapped and subject so sneeze, splash, dirt and dust.

Shelby Crossing Golf Course, 3885 Laser Road, Shelby, Oct. 21. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Ready-to-eat hot dogs in Pepsi cooler were not marked with date of opening. Direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed (critical, corrected). Drain for ice bin in bar was below flood rim.

Cinemark 14, 2355 Walker Lake Road, Ontario, Oct. 21. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed containers of popcorn and seasoning in cabinet below popcorn popping machines to be uncovered/not protected from contamination. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed opened bag of hot dogs in cooler without date mark. PIC discarded bag of two hot dogs.

Pleasant Hill Golf Course, 4089 Pleasant Hill Road, Perrysville, Oct. 21. Improper washing of hands and arms (critical). Worker observed using washed hands to touch contaminated surface of sink handle. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical repeat, corrected during inspection). Gallon of opened milk observe stored in basement cooler for over 24 hours without datemarking. Listed item was voluntarily discarded.

National Parkway Duke/Duchess/BP1186, 1020 National Pkwy., Mansfield, Oct. 24. PIC unable to demonstrate knowledge by having no critical violations (critical, corrected during inspection). Person in charge did not ensure employees are properly cooking TCS foods and/or using thermometers as required (critical, corrected). Employee observed removing foods from oven, preparing items for hot holding and not taking a temperature. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected). Employee engaged in food preparation was not informed in a verifiable manner of their responsibility to report. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Sugar in coffee condiment area had missing lids. PIC removed from service until lids could be obtained. Food equipment surfaces not cleaned at required frequency (critical, corrected). Tongs used for removing hot dogs were stored in contact with equipment and were not cleaned every four hours. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Lunch meat that was opened over 24 hours before was not date marked. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Creamer and cold brew coffee that stated that it contained milk was not date marked for seven days out. Food thermometers are not accurate (critical, corrected). Probe thermometer was not able to measure within plus or minus two degrees at time of taking internal temperatures for food. Food contact surfaces not easily cleanable (critical, corrected). Scissors used to open food bags were not able to be separated to allow proper cleaning and sanitizing of surface.

Dunkin' Donuts, 750 W. Ohio 97, Bellville, Oct. 24. PIC unable to demonstrate knowledge by having no critical violations (critical, corrected during inspection). PIC unable to demonstrate knowledge of cleaning and sanitizing (critical, corrected). PIC states in-use utensils that were observed to be in contact with TCS food and held in a metal container without temperature control are cleaned at end of day, not required minimum of every four hours. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected). Observed no signed employee illness agreements for food employees currently working in facility at time of inspection. Food not properly protected from contamination by separation, packaging, and segregation (critical, corrected). Observed ice storage bins, drink powders, milk jugs in reach in coolers below coffee prep areas, and tray of donuts uncovered/mot protected from possible contamination. TCS foods not being hot held at the proper temperature (critical repeat, corrected). Observed two sausage patties in warmer tray to be 107 degrees. PIC discarded. TCS foods not being cold held at the proper temperature (critical, corrected). Observed two cans each of pumpkin spice whipped cream, whipped cream and sweet cold foam stored on counter with internal temperatures of 55-58 degrees. Thirty eggs patties in prep cooler with internal temperature of 63 degrees. PIC discarded all listed food items. Improper storage of poisonous or toxic materials (critical, corrected). Observed boxes of Dawn dish soap and Spic & Span cleaner to be sitting on bakery prep table.

Marco's Pizza #1085, 1305 Lexington Ave., Mansfield, Oct. 24. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected during inspection). Sanitizer solution in the three-compartment sink was below 200 ppm. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Container of sliced American cheese in the pizza make table did not have a date mark. PIC voluntarily discarded cheese.

John Sherman Elementary, 1138 Springmill Road, Mansfield, Oct. 24. Warewashing machine not equipped with thermometer as specified (critical, corrected during inspection). Thermometer provided. Manual wash solution not being maintained at 110 degrees or above (critical, corrected). Wash solution measured at 100 degrees.
