Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between May 25 and June 1:

East Of Chicago Pizza Co., 11 W. Main St., Lexington, May 25. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Observed an opened container of beef in the pizza make table cooler without a date mark. Person in charge (PIC) voluntarily discarded approximately one pound of beef. No air gap or approved backflow prevention device on the plumbing system (critical repeat, corrected). Observed a black hose connected to the mop sink faucet without a vacuum breaker.

Meijer Express #126 Gas Station/C-Store, 1321 N. Lexington Springmill Road, Ontario, May 25. Improper storage of poisonous or toxic materials (critical, corrected during inspection). Observed pocket lighters containing lighter fluid stored next to packaged crackers on shelf in back storage area. PIC moved lighters to lower shelf below food items. Missing and/or incomplete allergen declaration(s) (critical). Bulk food for customer self-service not properly labeled. Observed Boston cream doughnut in display case to have a maraschino cherry on top and missing from ingredients list for that doughnut. Also observed doughnuts with sprinkles and/or chopped nuts not included on ingredient list.

Sipe Concessions #2, 1582 S. Bridgewater Way, Mansfield, May 26. Evidence of live rodents, and other pests (critical, corrected during inspection). Observed droppings in lower cabinets and on floor and lower storage shelves.

Shell Gas Station, 77 S. Diamond St., Mansfield, May 26. Presence of live insects, rodents, and other pests (critical, corrected during inspection). Observed flies in kitchen facility.

Twin Lakes Café, LLC DBA Twin Lakes Lounge, 2220 Marion Ave Road, Mansfield, May 26. PIC unable to demonstrate knowledge by having no critical violations (critical). Unsafe, adulterated or not honestly presented food not discarded or properly reconditioned (critical, corrected during inspection). Observed molded bread in walk-in cooler. PIC discarded at time of inspection. Presence of live rodents (critical, corrected during inspection). Observed mouse droppings on top of toaster oven and along wall under prep counter and handwashing sink.

Bloomin Tasty Tater, 77 Mill St., Lexington, May 26. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed residue on can opener blade. Evidence of live rodents, and other pests (critical, corrected). Observed droppings in lower cabinets and in oven.

Richland Skyline Inc., 1320 N. Lexington Springmill Road, Mansfield, May 26. Improper storage of poisonous or toxic materials (critical, corrected during inspection). Observed bottles of Orange Glo and cans of CLR cleaner on shelf above hot sauce. No air gap or approved backflow prevention device on the plumbing system (critical, corrected). Observed no air gap at Pepsi fountain machine.

Otterbacher Food Service, 20 High St., Shiloh, May 27. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). PIC unable to locate written procedures. Left a copy of RPH's employee illness reporting agreement form.

Mansfield Area Y, 750 Scholl Road, Mansfield, May 27. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical, corrected during inspection). Observed a bucket of bleach water in the three compartment sink at 0 ppm.

Long John Silver's #7179, 1200 Park Avenue West, Mansfield, May 31. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed a container of fish in the walk-in freezer that was uncovered.

Taco Bell #24647, 2085 Walker Lake Road, Mansfield, May 31. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed sour cream in stainless steel dispenser along cook line at 51 and 47 degrees. PIC discarded.

Oak Tree Golf Club LTD/J&C Golf Services LLC, 1440 Old Oak Trail, Mansfield, May 31. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical). Observed working sanitizer at 150 ppm. Quat sanitizing solution should be between 200-400 ppm.

Buns On The Run, 138 Vennum Ave., Mansfield, June 1. Working containers of poisonous or toxic materials not properly labeled (critical, corrected during inspection). Observed a spray bottle of cleaner without a common name on the label.

Charleys Philly Steaks, 2140 W. Fourth St., Ontario, June 1. PIC unable to demonstrate knowledge of temperatures and times for safe refrigerated food storage, hot holding, cooling and reheating (critical). Person in charge did not ensure that employees observed foods as they were received (critical). PIC states employees are not ensuring food is delivered at proper temperatures upon delivery. Correct by June 8. Food not properly protected from contamination by separation, packaging, and segregation (critical, corrected during inspection). Observed working utensils (tongs) that handle raw red meat and raw vegetables to be in contact with each other. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected). Observed working container of sanitizer at food prep area to be at 150 ppm. Improper cooling of TCS food (critical, corrected). Observed cooked chicken wings in walk-in cooler between 43 - 45 degrees that had been cooling for three days. PIC voluntarily discarded 40 pounds of chicken wings.

Mansfield Engineered Components-Mkt C, 1776 Harrington Memorial Road, Mansfield, June 1. TCS foods displayed in a micro market not discarded after the automatic shutoff was activated (critical, corrected during inspection). Chili chicken, pocket pizza, red hot beef and chimichangas with expired dates were voluntarily discarded. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Pepperoni/ mozzarella roll observed not date marked; discarded. TCS foods displayed in a micro market not discarded after the automatic shutoff was activated (critical, corrected). One Ham and Swiss and seven cheeseburgers with expired date marks were voluntarily discarded.

Speedway #3652, 564 N. Trimble Road, Mansfield, June 1. PIC did not ensure employees are properly maintaining temperatures of TCS foods during hot and cold storage (critical, corrected during inspection). TCS items in walk-in dairy cooler had internal temperatures of 48 - 55 degrees. PIC voluntarily discarded items. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected). Employee preparing food had not been informed in a verifiable manner of their duty to report. Food employee(s) did not wash hands when required (critical, corrected). Food employee observed changing tasks, contacting door handles and surfaces, without changing gloves and washing hands. Food not properly protected from contamination by separation, packaging, and segregation (critical). Two bread buns observed contacted by gloved hands that had contacted handles of hot holding units and fan. PIC voluntarily discarded buns. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Unwrapped apples observed in self-service grab-and-go fruit display exposed to possible contamination by consumer. Raw shell eggs not maintained at 45 degrees or below (critical, corrected). Ambient temperature of unit was at 56 degrees at time of inspection and above 45 degrees for an undetermined amount of time. PIC voluntarily discarded. TCS foods not being cold held at the proper temperature (critical, corrected). Items in walk-in cooler internal temperatures of 46 - 55 degrees for an undetermined amount of time. PIC voluntarily discarded. PIC discontinued use until unit can hold internal temperatures of 41 degrees or below.
