Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between Dec. 20 and Dec. 22:

Subway No. 25770, 14 Mill Road, Bellville, Dec. 20. Manual wash solution not being maintained at 110 degrees or above (critical corrected during inspection). Three compartment sink wash solution was measured at 83 degrees.

Ontario Estates Senior Living, 2010 Walker Lake Road, Mansfield, Dec. 20. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed tray of individually portioned fruit cups in reach-in cooler not covered/protected from possible contamination. TCS foods not being cold held at the proper temperature (critical, corrected). Observed gallon of whole milk in reach-in cooler to have internal temperature of 53 degrees. Person in charge (PIC) discarded milk during inspection. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed macaroni salad stored in walk-in cooler that was made in house not date marked. PIC verified with employee that the pasta salad was made the day prior. Also observed chocolate milk in stand up reach-in cooler not date marked. PIC unable to verify when it was opened/made and discarded. Food contact surfaces not easily cleanable (critical, corrected). Observed multiple rubber spatulas with broken and cracked edges in clean utensil storage. PIC removed from service and discarded.

Bob Evans Restaurants LLC No. 62, 0525 N. Trimble Road, Mansfield, Dec. 20. Handwashing sink is being used for purposes other than handwashing (critical, corrected during inspection). Observed coffee inside the hand sink next to the coffee makers. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Observed boxes of raw pork sausage on a shelf above raw pork bacon inside the walk-in cooler. TCS foods not being hot held at the proper temperature (critical, corrected). Mashed potatoes that had recently been placed on the make line were at at 120 degrees. PIC had mashed potatoes reheated to above 165 degrees.

I-71 & Ohio 97 Duchess No. 1188, 860 Ohio 97, Bellville, Dec. 21. PIC unable to demonstrate knowledge by having no critical violations (critical, corrected during inspection). Multiple critical violations observed. Education and corrective action provided. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical). No employee illness reporting agreements observed. Ensure food employees have signed by Dec. 28. Handwashing sink not accessible (critical, corrected). Observed both hand washing sinks to have items (utensils, empty jugs, etc.) sitting in them. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical, corrected). Observed working container of bleach on retail coffee counter to be at 200 ppm. PIC discarded bleach solution. TCS foods not being cold held at the proper temperature (critical, corrected). Observed TCS foods in retail grab-and-go cooler, as well as TCS creamers in creamer dispenser in retail area with internal temperatures of above 41 degrees for undetermined amount of time. PIC discarded TCS foods.

Mansfield Correctional Institution/Camp, 1150 N. Main St., Mansfield, Dec. 21. Manual wash solution not being maintained at 110 degrees or above (critical, corrected during inspection). Wash solution in three-compartment sink measured at 106 degrees.

Mansfield Correctional Institution/Main, 1150 N. Main St., Mansfield, Dec. 21. Food employee(s) did not wash hands when required (critical, corrected during inspection). Food worker observed donning new gloves without first washing hands. Handwashing sink is being used for purposes other than handwashing (critical repeat, corrected). Hand wash station on north side had ice observed inside sink. TCS foods not being hot held at the proper temperature (critical, corrected). Hot holding rice measured at 125 degrees. PIC voluntarily discarded pan of rice. Food contact surfaces not easily cleanable (critical repeat, corrected). Pots and pans observed with dents and were no longer smooth and cleanable. PIC stated there is a schedule in place to replace pots and pans that have been dented.

The Old Bag of Nail Pub - Mansfield LLC, 625 Lexington Ave., Mansfield, Dec. 22. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed an uncovered pan of cake inside the walk-in cooler. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed containers of bean dip in the walk-in cooler that were not date marked.

Pretzelmaker (Richland Mall), 2208 Richland Mall, Space No. C-8, Mansfield, Dec. 22. TCS foods not being hot held at the proper temperature (critical, corrected during inspection). Observed six containers of cream cheese in hot holding unit to have internal temperatures of 103-123 degrees. PIC discarded cream cheese. Food contact surfaces not easily cleanable (critical). Observed baking sheets used to line metal pans for pretzels to be torn and no longer easily cleanable. Discontinue use of baking sheets or replace by Dec. 29. Insufficient air gap between the flood rim and the water supply inlet (critical). Observed sprayer head at three-compartment sink to hang below flood rim level of sink. Correct by Dec. 29.

Big Fish General Store & Bait LLC, 3911 Ohio 95, Perrysville, Dec. 22. Improper washing of hands and arms (critical, corrected during inspection). Worker observed using washed hands to touch contaminated handle surface to turn off water. Ready-to-eat, TCS food not properly discarded when required (critical, corrected). Cold-holding salami with discard date of Dec. 14 not discarded. Ready-to-eat, TCS food not properly discarded when required (critical, corrected). Cold-holding pepperoni with discard date of Dec. 21 not discarded.

Ontario Pointe, 2124 Park Avenue West, Mansfield, Dec. 22. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed milk on serving cart with internal temperature of 45 degrees. PIC placed milk back in cooler. Observed bowl of coleslaw at steam table in kitchen to have internal temperature of 56 degrees, bottle of thousand island dressing to have internal temperature of 68 degree and sliced provolone cheese at 43 degrees. PIC discarded both the coleslaw and dressing and placed cheese back in cooler to bring back down to 41 degrees. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed a package of deli turkey and container of potato salad in cooler without a date mark. PIC unable to verify when turkey was opened and discarded package. PIC was able to verify when potato salad was opened/placed into another container and date marked during inspection.
