Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted Feb. 8 and Feb. 9:

Brown Derby Roadhouse, 3985 Park Avenue West, Ontario, Feb. 8. Food not properly protected from contamination by separation, packaging, and segregation (critical, corrected during inspection). Observed bottom of pan sitting directly on foods in the hot pit meat cooler. Person in charge (PIC) voluntarily discarded one piece of steak. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed tongs in the salad bar to be hanging on a pan with food contact surfaces touching the wall. Presence of live insects (critical repeat, corrected). Observed small black flying insects around the server stations, under the warewashing unit and near the onion racks. PIC contacted pest control provider. Remove all evidence, clean the area and monitor the activity by re-inspection Feb. 15.

Fresh Grounds Coffee House LLP, 1621 W. Fourth St., Ontario, Feb. 8. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed individual serving cups of house made vinaigrette on service counter at room temperature. PIC placed in cooler to cool to 41 degrees or below. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed house made vinaigrette not date marked. Also observed cut lettuce and cooked chicken in prep cooler date marked for longer than 7 total days. All had been prepped day of inspection or day prior and PIC date marked correctly.

Kroger Company #N836, 1500 Lexington Ave., Mansfield, Feb. 8. Presence of live insects (critical repeat). Observed small black flying insects in the drain areas of the deli and Starbucks. Have removed by re-inspection.

The Butcher Shoppe, 279 S. Diamond St., Mansfield, Feb. 8. PIC unable to demonstrate knowledge of cleaning and sanitizing (critical, corrected during inspection). PIC stated the deli meat and cheese slicer is cleaned once at the end of the day. Discussed breaking down slicer for cleaning at least every four hours. No safe handling instructions on packaged raw meat in the retail freezer (critical repeat, corrected). Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected). Observed uncovered meat on the shelf inside the walk-in cooler. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected)

Observed partially cooked smoked turkey necks on a shelf above milk and yogurt in the retail foods cooler. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed an opened package of ham in the deli case without a date mark. Incomplete HACCP plan (critical). Facility is vacuum-packaging raw meat without a HACCP plan. Correct by Feb. 16. No air gap or approved backflow prevention device on the plumbing system (critical). Observed a hose without a backflow device below the three-compartment sink. Correct by Feb. 16.

Cheddar's Casual Café 2011, 979 N. Lexington Springmill Road, Mansfield, Feb. 8. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected during inspection). Observed container of lemons in servers area and container of frozen shrimp in reach-in cooler uncovered/unprotected from possible contamination. TCS foods not being cold held at the proper temperature (critical, corrected).

Observed containers of refrigerated mixes in bar area and finest call syrups with internal temperatures between 45-47 degrees. PIC voluntarily discarded listed mixes. Water source and/or hot water capacity not sufficient (critical, corrected). Observed water temperature at faucet of three-compartment sink in bar area to only be 100 degrees. Increase hot water temp by Feb. 15.

Empress Express, 1041 Park Avenue West, Mansfield, Feb. 8. Person in charge not ensuring proper cooling (critical, corrected during inspection). PIC unable to answer time and temperature parameters for proper cooling. Specialist provided education at time of inspection. PIC unable to demonstrate knowledge by having no critical violations (critical). Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected). Observed employee health agreements for food handling employees not signed. Food employee(s) did not wash hands when required (critical, corrected). Observed food employees not washing hands between tasks. Foods from unapproved sources (critical, corrected).

Observed unlabeled 20 pints of hot chili sauce being offered for sale that was canned in the home of an employee. Employee removed sauce from facility. No home produced/canned foods of any kinds are to be offered or served for human consumption in a licensed facility. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected). Observed several items uncovered in the walk-in and the reach-in cooler by the wok. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical repeat, corrected). Observed sanitizer bucket by the cash register with a concentration of 0 ppm. Improper storage of poisonous or toxic materials (critical, corrected). Observed several chemical bottles stored above foods. Working containers of poisonous or toxic materials not properly labeled (critical). Observed several squirt bottles with chemical unlabeled. Correct immediately.

Presence of live rodents (critical). Observed rodent droppings in the back storage area around and behind the catering materials, small back storage room around the hot water heater, and the little nook near the bakery area. Contact licensed pest control provider by re-inspection Feb. 15. Manual wash solution not being maintained at 110 degrees or above (critical, corrected). Observed warewashing water to be 102 degrees. Insufficient air gap between the flood rim and the water supply inlet (critical). Observed ice machine to have an insufficient air gap. Correct by Feb. 15.

Planktown Country Market LLC, 1921 Free Road, Shiloh, Feb. 9. PIC unable to demonstrate knowledge by having no critical violations (critical). Correct by re-inspection. Presence of live rodents (critical repeat). Mouse dropping observed in retail area aisle 12 on and behind shelves, windowsill and floor plus spice room and storage area behind pallets. Clean all these areas, removing all evidence of activity, and contact pest control company. Correct by re-inspection.

7- Eleven Store #36174 A, 2264 S. Main St., Mansfield, Feb. 9. Handwashing sink not accessible (critical, corrected during inspection). Observed back hand sink with beer cans, etc. stored within sink basin.

A Wok, 1408-A Lexington Ave., Mansfield, Feb. 9. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected during inspection). Observed raw beef stored over water for soups. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected). Observed uncovered thawing beef and water for soups in the walk in cooler. TCS foods not being cold held at the proper temperature (critical, corrected). Observed one-half pound of cut cabbage on prep table with internal temperature of 64 degrees. PIC voluntarily discarded cabbage. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed cream cheese in the walk-in with no date mark.

Chinatown, 283 Ashland Road, Mansfield, Feb. 9. Equipment food-contact surfaces or utensils are unclean (critical). Observed can opening blade that has residue and loss its plating. Replace blade; correct by Feb. 13.
