Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between Dec. 16. and Dec. 19:

In-N-Out Mart #103, 300 N. Mulberry St., Mansfield, Dec. 16. Handwashing sink not accessible (critical, corrected during inspection). Observed a beer can stored in hand wash sink.

Warrior Drive-In & Pizza, 3393 Park Avenue West, Mansfield, Dec. 16. Chemical sanitizer other than chlorine, iodine, or a quaternary ammonium applied improperly (critical, corrected during inspection). Observed sink and surface sanitizer in three-compartment sink and sani bucket to be under strength. Observed automatic dispensing unit to not be dispensing at proper strength. Person in charge (PIC) contacted company for service request and switched to using chlorine sanitizer manually until auto dispenser can be fixed. Manual wash solution not being maintained at 110 degrees or above (critical, corrected). Observed employee washing equipment/utensils in three-compartment sink with water temperature at 98 degrees.

Burger King Restaurant #396, 2242 S. Main St., Mansfield, Dec. 16. PIC unable to demonstrate knowledge of cleaning and sanitizing (critical, corrected during inspection). PIC states the frosty machine blender portion is sanitized every four hours but not washed and rinsed prior. Reviewed properly cleaning procedures. Food employee(s) did not wash hands when required (critical, corrected). Observed PIC touch floor and face without washing hands before starting another task. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Observed lid to ice bin in drive thru left open when not in use. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Observed cooked bacon not covered to protect from contamination. Equipment food-contact surfaces or utensils are unclean (critical). Observed clean sauce bottles above three-compartment sink to contain staining on food contact surfaces. Correct by Dec. 23. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected). Observed sanitizer solution at three-compartment sink at 0 ppm. Observed sanitizer bag empty. TCS foods not being cold held at the proper temperature (critical, corrected). Observed one pack of sliced ham and one bag of shredded cheese in the top of a prep cooler not properly cold held. PIC voluntarily discarded product. Ready-to-eat, TCS food not properly discarded when required (critical, corrected). Observed a pan of tomatoes on the make line dated to be used by Thursday. PIC voluntarily discarded product. Improper use of time as a public health control - four hours (critical, corrected). Observed tomatoes and cheese not time marked with four hour discard time. Tomatoes were discarded and cheese was time stamped. Observed products not being marked with time they were pulled from cooler. Evidence of rodents, and other pests (critical). Observed droppings under beverage station in dining area. Correct by Dec. 23. Food contact surfaces not easily cleanable (critical). Observed scissors used to cut open food bags not able to separate for easy cleaning. Correct by Dec. 23. Manual wash solution not being maintained at 110 degrees or above (critical, corrected). Observed the wash solution at the three-compartment sink at 87 degrees. No air gap or approved backflow prevention device on the plumbing system (critical). Observed a funnel between the ice machine drain and floor drain with no proper air gap. Correct before Dec. 23.

Lexington Eastern Elementary, 155 Castor Drive, Lexington, Dec. 16. Ready-to-eat, TCS food not properly discarded when required (critical, corrected during inspection). Egg yolks observed stored in walk-in cooler with expired date of Nov. 29. PIC voluntarily discarded.

Domino's Pizza, 625 Lexington Ave., Mansfield, Dec. 19. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical repeat). Observed no employee illness agreements signed at time of inspection. Ensure agreements are signed and accessible by re-inspection Dec. 27. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected during inspection). Observed sanitizer at both the warewashing sink and the bucket by the line to be at 150 ppm. Ensure proper concentration until dispersal machine can be serviced. Presence of live insects (critical). Observed small black flying insects in the warewashing area. Have serviced by Dec. 27. Food contact surfaces not easily cleanable (critical, corrected). Observed spatula broken and chipped, rendering it not longer smooth, durable and easily cleanable. PIC removed the spatula from service. Food contact surfaces not easily cleanable (critical repeat, corrected). Observed scissors being used to open food packages that are not smooth and easily cleanable between the blades. PIC discontinued use of scissors until pair that can be easily disconnected and cleaned are obtained.

McDonald's - Richland Mall #3350, 666 Lexington Springmill Road, Mansfield, Dec. 19. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical, corrected during inspection). Observed working sanitizer bucket to be at 200 ppm. TCS foods not being cold held at the proper temperature (critical, corrected). Observed breakfast burritos in cooler to have internal temperature of 48 degrees and ambient air temp of cooler to be at 53 degrees. PIC voluntarily discarded all breakfast burritos (46) during inspection and discontinued use of cooler until maintenance can fix.

NC State/OSU-M Child Development Center, 2441 Kenwood Circle, Mansfield, Dec. 19. Food package(s) received in poor condition (critical, corrected during inspection). Observed cans of tomato soup and whole kernel corn with dents along top and bottom seals. PIC removed cans from stock to be returned. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed blade on electric can opener to contain visible residue.

Subway #30929, 1521 Lexington Ave., Mansfield, Dec. 19. Presence of rodent droppings and small flying insects (critical repeat). Observed rodent dropping on the floors and corners of the mop sink room. Observed rodent dropping also in the electrical room, on the floor and on the wall-mounted electrical panels. Observed small black flying insects in the warewashing area. Clean entire area and contact pest control company by re-inspection Dec. 27. Insufficient air gap between the flood rim and the water supply inlet (critical repeat). Observed no air gap under the soda dispensing machine. Correct by re-inspection Dec. 27.

Wendy's Restaurant, 2450 O'Possum Run Road, Mansfield, Dec. 19. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Observed onions not properly date marked in walk-in cooler. PIC removed onions.
