Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between Oct. 25 and Oct. 31:

Gionino's Pizzeria, 176 B E. Main St., Lexington, Oct. 25. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed uncovered prepared pizzas in the walk-in cooler.

7-Eleven Inc. (a Texas Corporation) & Pria, Inc. Store # 36177 A, 876 Park Avenue East, Mansfield, Oct. 25. Quaternary ammonium sanitizing solution at incorrect temperature, concentration and/or water hardness (critical). Observed no sanitizer available at three-compartment sink. Correct by Oct. 27. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed milk/French vanilla dispenser reading 58 degrees. Person in charge (PIC) discarded containers.

Taco Bell # 35253, 1423 Lexington Ave., Mansfield, Oct. 26. Manual wash solution not being maintained at 110 degrees or above (critical). Observed the active washing solution to have a temperature of 105 degrees. PIC added more hot water to bring temperature back up to 110 degrees.

Dunkin #556, 750 W. Ohio 97, Bellville, Oct. 26. Handwashing sink is being used for purposes other than handwashing (critical, corrected during inspection). Observed blenders in the hand washing sink. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed residue on the inside of the hot chocolate dispenser. Quaternary ammonium sanitizing solution at incorrect temperature, concentration and/or water hardness (critical repeat, corrected). Observed solution in sanitizer buckets at 0 ppm. Improper storage of poisonous or toxic materials (critical, corrected). Observed cleaning chemicals on a shelf above a box of lids in the storage shed.

Clear Fork High School, 987 Ohio 97, Bellville, Oct. 26. PIC did not ensure employees are properly maintaining temperatures of TCS foods during hot and cold storage (critical, corrected during inspection). Cooler on serve line was observed registering temperatures below 51-59 degrees. TCS foods relocated to walk-in cooler. TCS foods not being cold held at the proper temperature (critical, corrected). Ready-to-eat tomatoes on serve line was measured at 46 degrees. Tomatoes placed in cooler. TCS foods not being cold held at the proper temperature (critical, corrected). Cooler on serve line observed with ambient temperatures below 51-59 degrees; internal temperature of tomatoes measured at 51 degrees. Listed items were relocated and cooler setting adjusted.

Speedway #3535, 838 Ohio 97, Bellville, Oct. 26. Equipment food-contact surfaces or utensils are unclean (critical, connected during inspection). Observed residue on the upper portion inside the ice chutes at the fountain drinks machine.

St. Mary's Catholic School, 1630 Ashland Road, Mansfield, Oct. 27. Insufficient air gap between the flood rim and the water supply inlet (critical, corrected during inspection). Hose on spigot of three-compartment sink stored next to drain. Hose removed to create air gap.

Wilging's Fine Meats, 1391 Lexington Ave., Mansfield, Oct. 27. PIC unable to demonstrate knowledge by having no critical violations (critical, corrected during inspection). PIC not ensuring employees are properly sanitizing equipment and utensils and monitoring of sanitizer parameters (critical, corrected). Observed employee wash, rinse and place dish on towel to dry. Ensure all employees are allowing dishes to air dry on a smooth, durable, non-absorbent surface. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical, corrected). Observed chlorine sanitizer concentration in a bucket to be more than 200 ppm. TCS foods not being hot held at the proper temperature (critical, corrected). Observed baked beans that were placed into the hot well 10 to have an internal temperature of 105 degrees. Ready-to-eat, TCS food not properly discarded when required (critical repeat, corrected). Observed macaroni salad to have a date mark of Oct. 19. PIC discarded at time of inspection.

Little Blessings N More LLC, 250 Glessner Ave., Mansfield, Oct. 27. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed an uncovered tray of milk cups in the cooler.

Ontario Duchess #1249, 3675 Park Avenue West, Ontario, Oct. 30. TCS foods not being cold held at the proper temperature (critical repeat, corrected during inspection). Observed TCS foods in retail deli cooler held above 41 degrees. Italian subs and cut pepper cheese placed in walk-in cooler to cool back down to 41 degrees or less.

Madison Eastview Elementary, 1262 Eastview Dr., Mansfield, Oct. 30. Unnecessary persons in the operation (critical, corrected during inspection). Teacher observed walking into kitchen warewashing area and dry goods room.

Nickel & Bean, 27 E. Main St., Lexington, Oct. 31. Person in charge did not ensure that employees observed foods as they were received (critical, corrected during inspection). PIC stated that no temperatures of products received are being taken. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Observed scones in the café area and cinnamon rolls in the bakery freezer that were uncovered. Ready-to-eat, TCS food not properly discarded when required (critical, corrected). Observed Colby jack cheese and smoked cheddar cheese with a date mark of Oct. 19 in the bakery. PIC voluntarily discarded cheese.

Lexington Senior Civic Center, 67 E. Main St., Lexington, Oct. 31. Ready-to-eat, TCS food not properly discarded when required (critical, corrected). Observed containers of nacho cheese in the cooler with an opened date of Oct. 9. PIC removed nacho cheese containers from the cooler.

Lexington Court Care Center, 250 Delaware St., Lexington, Oct. 31. Hot water for mechanical sanitization is below required temperature (critical). Observed the utensil surface temperature 150 degrees. Discussed using a chemical sanitizer solution for dishes until dishwasher is repaired. Correct by Nov. 7. Insufficient air gap between the flood rim and the water supply inlet (critical). Observed the ends of the ice machine drain pipes down in the funnel around the floor drain eliminating the air gap. Correct by Nov. 7.

St. Peter's Catholic High School, 104 W. First St., Mansfield, Oct. 31. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Ready-to-eat lettuce in walk-in cooler observed on second shelf uncovered and subject to sneeze, splash, dirt and dust. TCS foods not being cold held at the proper temperature (critical, corrected). Lettuce observed on food line not submerged into ice and measured at 46 degrees. Lettuce was taken off food line and placed in cooler. New lettuce was observed submerged in ice with smaller container.

Coney Island, 20 S. Park St., Mansfield, Oct. 31. TCS foods not being cold held at the proper temperature (critical). Observed walk-in cooler that was reading 46 degrees. Remove perishable food items ASAP. Correct by Nov. 2.
