Richland County restaurant inspections: critical violations, Feb. 2-7, 2024

The following restaurant inspections with critical violations were conducted by Richland Public Health between Feb. 2 and Feb. 7:

Tamara K. King (Bostic Time), 4218 Park Ave. West, Mansfield, Feb. 2. Person in charge (PIC) unable to demonstrate knowledge by having no critical violations (critical repeat). Multiple critical violations observed. Correct by Feb. 9. Handwashing sink not accessible (critical repeat). Facility observed to have no running water rendering handwashing sinks inaccessible. At re-inspection, observed hand washing sink not functioning due repair issues. Correct by Feb. 9. Private water system does not meet required water quality standards (critical). Facility observed to have obtained a new well but does not have a passing water sample and therefore has not met the water quality standards established in Chapter 3701-28 of the Administrative Code. Facility continues to utilize disposable single use articles only and purchasing ice used for mixed drinks. Facility to contact Richland Public Health with disinfection record and to schedule a water sample. Correct by Feb. 9. No air gap or approved backflow prevention device on the plumbing system (critical repeat). No backflow observed on plumbing system for facility. Correct by Feb. 9.

Wingate By Wyndham, 2069 Walker Lake Road, Ontario, Feb. 2. No air gap or approved backflow prevention device on the plumbing system (critical repeat). No backflow device observed on the water supply for the Bunn coffee maker in the kitchen area. Correct by Feb. 12.

Hamloaf Inc. DBA V&M Restaurant, 1120 Lexington Ave., Mansfield, Feb. 5. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed cartons of raw eggs inside the coolers above ready-to-eat foods. Improper storage of poisonous or toxic materials (critical, corrected). Observed a container of disinfecting wipes resting on a box of pudding mix on the shelf in the hallway.

Speedway #3624, 2132 W. Fourth St., Mansfield, Feb. 5. Equipment food-contact surfaces or utensils are unclean (critical). Observed visible residues on roller grill in dining area. PIC states roller is cleaned daily during third shift. Discussed ensuring roller is thoroughly cleaned to remove visible residues. Correct by Feb. 15. Improper storage of poisonous or toxic materials (critical, corrected during inspection). Observed multiple containers of white vinegar stored on shelf with cleaning chemicals. Presence of live insects (critical repeat). Observed small flying insects near retail coffee and fountain beverage counters, as well as ants inside cabinet at coffee counter. Correct by Feb. 15.

Rancho Fiesta, 1360 S. Trimble Road, Mansfield, Feb. 5. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected during inspection). Observed various foods in the reach-in cooler that were uncovered. TCS foods not being cold held at the proper temperature (critical, corrected). Observed steak, carnitas, cheese and tilapia to be above acceptable temperature parameters of 41 degrees or below in the reach-in cooler. PIC voluntarily discarded the tilapia and reconditioned the steak and carnitas.

Wendy's Restaurant, 2450 O'Possum Run Road, Mansfield, Feb. 5. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed eggs holding at 53 degrees. Eggs were discarded during inspection.

Blueberry Patch Greenhouse & Gifts, 1285 W. Hanley Road, Mansfield, Feb. 6. Ready-to-eat, TCS food not properly discarded when required (critical repeat, corrected during inspection). Observed TCS foods (stuffed pepper soup, feta cheese and house made lemon vinaigrette) held longer than seven days. PIC had all listed items voluntarily discarded. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical repeat, corrected). Observed cooked sausage in top of prep cooler not date marked. PIC verified sausage was cooked Feb. 4 but was voluntarily discarded. Also observed pan of corn chowder not marked for when bag was opened. PIC verified soup was opened on Feb. 2 and date marked during inspection.

Mickey Mart #22, 189 Plymouth St., Plymouth, Feb. 6. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected during inspection). Observed pizza butter brush to be uncovered, rendering it exposed to sneeze, splash and dust. Improper display of poisonous or toxic materials offered for retail sale (critical, corrected). Observed soap and spackling stored over drinks for sale in the storage area. Ensure all chemicals are stored below or completely separated from foods for sale.

Steve's Dakota Grill, 3101 Park Ave. West, Mansfield, Feb. 6. PIC unable to demonstrate knowledge by having no critical violations (critical repeat, corrected during inspection). Handwashing sink is being used for purposes other than handwashing (critical, corrected). Observed pair of tongs hanging on faucet of handwashing sink in the kitchen. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected). Observed raw potatoes being stored under raw meat in the walk-in cooler. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed knife blade to have visible residue. Also observed clean stored skillet to be in contact to chemical dispensing unit, rendering the pan no longer clean and free of contamination. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed feta cheese to be without date mark. Ready-to-eat, TCS food not properly discarded when required (critical, corrected). Observed house made sauces (cocktail, BBQ, cinnamon butter and horsey), Mediterranean vegetables and hot country vegetables to be passed date mark. PIC voluntarily discarded all items. Food contact surfaces not easily cleanable (critical, corrected). Observed cracked/chipped food contact portion of pitcher and spatula, rendering the surfaces no longer smooth, durable, non-absorbent, or easily cleanable. PIC voluntarily discarded both. Insufficient air gap between the flood rim and the water supply inlet (critical repeat, corrected). Observed no air gap under the ice machine.

Sam's Club #6407, 1070 N. Lexington Springmill Road, Mansfield, Feb. 7. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed uncooked pies on speed racks in bakery area not covered/protected. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected). Observed packages of uncooked turkey bacon on a shelf above fully-cooked ham steaks in the retail cooler. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected). Observed the sanitizer in the café three-compartment sink.
