Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health Nov. 3 through Nov. 9:

The Golden Burrito #2, 1740 Park Avenue West, Ontario, Nov. 3. Person in charge (PIC) not ensuring proper cooling (critical). PIC unable to demonstrate knowledge by having no critical violations (critical repeat). PIC did not ensure employees are properly maintaining temperatures of TCS foods during hot and cold storage (critical). PIC unable to demonstrate knowledge of temperatures and times for safe refrigerated food storage, hot holding, cooling and reheating (critical). Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical repeat, corrected during inspection). No employee illness reporting agreements observed for food employees working in kitchen at time of inspection. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Observed raw beef in metal pan sitting on top of a pan of cooked chicken in cooler. Also observed pan of cooked chicken uncovered in large reach-in cooler. Improper cooling of TCS food (critical, corrected ). Observed large pan of covered cooked shrimp cooling back prep table at room temperature. Internal temperature observed between 96-100 degrees. TCS foods not being hot held at the proper temperature (critical repeat, corrected). Observed metal pans of pinto beans sitting on shelf below steamer unit to be at 100 degrees. TCS foods not being hot held at the proper temperature (critical repeat, corrected). Observed TCS foods (chicken, pork) in metal pans on griddle to be below 135 degrees. TCS foods not being cold held at the proper temperature (critical, corrected). Observed multiple TCS foods (guacamole, cooked shrimp, salsa) on top make areas of prep coolers not being held at 41 degrees or less. Also observed metal pan of ground beef from small reach-in cooler to have internal temperature of 44 degrees. PIC voluntarily discarded shrimp and ground beef. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed multiple TCS foods (cooked pork, cooked chicken and cooked pasta) in cooler without date marking. Working containers of poisonous or toxic materials not properly labeled (critical, corrected). Observed spray bottle of floor cleaner in chemical storage area not labeled. Manual wash solution not being maintained at 110 degrees or above (critical repeat, corrected). Observed employee washing dishes and wash water observed to be 85 degrees. Insufficient air gap between the flood rim and the water supply inlet (critical). Observed sprayer head at three-compartment sink to hang level with flood rim of sink, no air gap. Correct by Nov. 10.

Spanish Immersion, 240 Euclid Ave., Mansfield, Nov. 4. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Tossed salad on serving line was measured at 51 degrees. Salad taken off serving line and placed in cooler. Next lunch shift salad measured at 40 degrees and placed in ice for cold holding on the line.

Mansfield Senior Culinary Arts, 124 N. Linden Ave., Room M1801b, Mansfield, Nov. 7. Handwashing sink not accessible (critical, corrected during inspection). Hand wash sink was blocked by mop bucket and cart. Food equipment surfaces not cleaned at required frequency (critical, corrected). Two washed pots observed hanging above three-compartment sink with food residue.

Garden Gateway Café By Buehler's, 50 Trimble Road, Mansfield, Nov. 8. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). Signed copies of the employee health policy are not kept on file in the facility. Food employee(s) did not wash hands when required (critical, corrected). Observed employee return to preparing food before washing hands after touching the cash register.

Wedgewood Estates, 600 S. Trimble Road, Mansfield, Nov. 8. PIC unable to demonstrate knowledge by having no critical violations (critical). Handwashing sink is being used for purposes other than handwashing (critical, corrected during inspection). Observed sanitizer bucket in the sink in the satellite kitchen #2. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Observed a carton of raw eggs in the walk-in cooler over a container of bacon bits. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical). Observed a concentration of 10 ppm of sanitizer solution at the warewashing unit. Correct by re-inspection on Nov. 15. TCS foods not being hot held at the proper temperature (critical, corrected). Observed cooked chicken in the hot holding unit to have a internal temperature of 113 degrees and some noodles in the same unit to have an internal temperature of 105 degrees. TCS foods not being cold held at the proper temperature (critical repeat, corrected). Observed American cheese and American Swiss cheese to have internal temperatures of 48-54 degrees. PIC had both discarded. Ready-to-eat, TCS food not properly discarded when required (critical, corrected). Observed package of American cheese to be past datemark (Oct. 30). PIC had cheese discarded. Ready-to-eat, TCS food not properly discarded when required (critical, corrected). Observed bone broth in the walk-in cooler to be past datemark (Sept. 27). PIC discarded at time of inspection.

Ontario Stingel Elementary, 416 Shelby Ontario Road, Mansfield, Nov. 8. PIC did not ensure employees are properly maintaining temperatures of TCS foods during hot and cold storage (critical, corrected during inspection). Lettuce at salad bar measured at 48 degrees. TCS foods not being cold held at the proper temperature (critical, corrected). TCS lettuce observed stored in pan outside cooling unit of salad bar unit registering over 48 degrees.

Subway #53308, 976 Ohio 97, Bellville, Nov. 9. PIC unable to demonstrate knowledge by having no critical violations (critical). PIC unable to demonstrate knowledge of cleaning and sanitizing (critical, corrected). When questioning PIC about warewashing procedures, it was discovered that facility is skipping the rinse step of the warewashing procedures. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical repeat). Observed no employee illness forms accessible at time of inspection due to being locked in the office. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed visible residue on the tomato slicer. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical, corrected). Observed mechanical dishwasher not dispersing the sanitizer. PIC discontinued use until the unit can be repaired or replaced. Manual warewashing in three-compartment sink will be utilized. Correct by next standard inspection. Food contact surfaces not easily cleanable (critical, corrected). Observed numerous pairs of scissors throughout the facility used for opening food packages that were not able to be taken apart and easily cleaned. Replace existing scissor with appropriate pair for food contact surfaces by Nov. 15.

Bellville Elementary School, 195 School St., Bellville, Nov. 9. PIC did not ensure employees are properly maintaining temperatures of TCS foods during hot and cold storage (critical, corrected during inspection). TCS lettuce on serve line was measured at 55 degrees. Item was voluntarily discarded. Unnecessary persons in the operation (critical). Three students and teacher aide walked into facility and brought crates of juice into walk-in cooler. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Single portions of sauce for breadsticks observed stored on top counter of sneeze guard where they were subject to sneeze, splash, dirt and dust near student clientele. TCS foods not being cold held at the proper temperature (critical repeat, corrected). Lettuce on serve line was measured at 55 degrees. Listed item was voluntarily discarded.
