Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between Jan. 12 and Jan. 20:

Newman Technology Inc., 100 Cairns Road, Mansfield, Jan. 12. Person in charge (PIC) unable to demonstrate knowledge of cleaning and sanitizing (critical). Not proper test papers and knowledge of sanitizing. Discussed violation with PIC. Handwashing sink not accessible (critical). Back hand wash sink was not accessible. Correct by Jan. 18. TCS foods not being cold held at the proper temperature (critical). Observed salad bar cooler reading 54 degrees. Action: Move food to cold holding unit or throw out immediately. Correct by Jan. 18. Water under pressure is not available to all fixtures as required (critical). Primary hand sink has low hot water pressure.

Subway Sandwich Shoppe #38123, 359 N. Lexington Springmill Road, Ontario, Jan. 14. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical repeat). Employee was unable to provide signed employee illness reporting agreements. Please have them where employee can find by Jan. 22.

Boardwalk Foods Inc., 98 N. Main St., Mansfield, Jan. 14. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed a container of diced ham on the bottom shelf underneath raw ground beef inside the sliding door cooler. PIC moved ham to the shelf below ready-to-eat foods inside the cooler. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Observed a container of chili in the cooler that had been prepared more than 24 hours prior without a date mark.

Lorain Music/Commercial Cutting - Lorain Music & Vending Co. Inc., 208 Central Ave., Mansfield, Jan. 14. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Cheeseburger datemarked Jan. 12 observed in food vending machine. Listed item was voluntarily discarded.

Hamloaf Inc. DBA V&M Restaurant, 1120 Lexington Ave., Mansfield, Jan. 18. Handwashing sink is being used for purposes other than handwashing (critical, corrected during inspection). Observed a pan sitting in the handwashing sink in the kitchen. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical repeat, corrected during inspection). Bucket of quat sanitizer solution at the front servers station was below 100 ppm. PIC refilled solution to a concentration of 200 ppm at the time of inspection. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed sliced ham in a container above the fill line in the make table cooler at 46 degrees. PIC voluntarily discarded ham. Refrigerated, ready-to-eat, TCS foods not properly datemarked (critical, corrected during inspection). Pan of baked potatoes held for more than 24 hours in the walk-in cooler did not have a datemark.

Mansfield Senior Culinary Arts, 124 N. Linden Ave., Room M1801b, Mansfield, Jan. 18. Improper temperature of wash solution in mechanical warewasher (critical repeat). EcoLab warewasher measured only 171 degrees (163 degrees on machine) when data plate stated warewasher should have temperatures of at least 180 degrees. Correct by Jan. 28.

Rancho Fiesta, 1360 S. Trimble Road, Mansfield, Jan. 18. PIC unable to demonstrate knowledge of temperatures and times for safe refrigerated food storage, hot holding, cooling and reheating (critical, corrected during inspection). PIC unable to state time and temperatures for cooling of food. Facility is to use cooling logs provided and maintain for sanitarian on re-inspection. Handwashing sink is being used for purposes other than handwashing (critical, corrected during inspection). Hand sink in bar had items in it at time of inspection. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected during inspection). Salsa in walk-in cooler observed without lids and not in use. Refrigerated, ready-to-eat, TCS foods not properly datemarked (critical repeat, corrected during inspection). Meat ball soup made over 24 hours ago did not have datemark.

Athena Greek Restaurant, 41 N. Main St., Mansfield, Jan. 19. Quaternary ammonium sanitizing solution at incorrect temperature, concentration and/or water hardness (critical, corrected during inspection). Wiping cloth bucket had no sanitizer detected. Solution was replaced.

Butler Clear Fork Adult Center, 20 Henry St., Butler, Jan. 19. TCS foods not being hot held at the proper temperature (critical, corrected during inspection). Mixed vegetables measured at only 124 degrees in hot holding unit. Item was reheated to over 165 degrees.

MVP Sports Bar & Grill, 275 Park Avenue West, Mansfield, Jan. 19. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed a container of raw chicken stored on the shelf above sliced cheese in the walk-in cooler. PIC moved chicken to a shelf below ready-to-eat foods. Ready-to-eat, TCS food not properly discarded when required (critical, corrected during inspection). Observed a container of homemade coleslaw in the Pepsi cooler with a prepared date mark of Jan. 8. PIC voluntarily discarded coleslaw. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Observed a bag of cooked sausage that had been held for more than 24 hours in the make table cooler without a date mark.

Five Guys Burgers & Fries, 994 N. Lexington-Springmill Road 160, Ontario, Jan. 19. TCS foods not being hot held at the proper temperature (critical, corrected during inspection). Cooked mushrooms observed hot holding at only 131 degrees. Listed item was reheated to 165 degrees. TCS foods not being hot held at the proper temperature (critical, corrected during inspection). Cooked onion measured hot holding at only 124 degrees. Listed item was reheated to above 165 degrees.

Butcher Shoppe, 279 S. Diamond St., Mansfield, Jan. 20. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). Provided Richland Public Health's employee illness reporting agreement form at the time of inspection. Unpasteurized eggs not labeled with safe handling instructions (critical). Correct by Jan. 27.
