Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between June 22 and June 23:

AMVETS Post #26 - Home Building Co., 1100 W. 4th St., Mansfield, June 22. Ready-to-eat, TCS food not properly discarded when required (critical repeat, corrected during inspection). Observed a bag of cantaloupe that was cut on June 13. Person in charge (PIC) voluntarily discarded cantaloupe at the time of inspection. Ready-to-eat, TCS food not properly discarded when required (critical repeat, corrected during inspection).

Observed a bag of hot dogs that were opened on June 10. PIC voluntarily discarded hot dogs. Ready-to-eat, TCS food not properly discarded when required (critical repeat, corrected during inspection). Observed a container of chicken salad with a prep date of June 6. PIC voluntarily discarded chicken salad.

Mansfield Memorial Homes, 50 Blymyer Ave., Mansfield, June 22. PIC not ensuring employees are properly sanitizing equipment and utensils and monitoring of sanitizer parameters (critical, corrected during inspection). Sanitizer strength in three-compartment sink was 0 ppm. PIC remade to strength of 200 ppm. Facility is manually mixing until dispenser can be serviced. PIC unable to demonstrate knowledge of temperatures and times for safe refrigerated food storage, hot holding, cooling and reheating (critical, corrected during inspection). PIC unable to state correct cooling temperatures and times. Handout provided at time of inspection.

Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected during inspection). Sanitizer in three-compartment sink and buckets at 0 ppm. PIC remade to correct strength. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Meat slicer had visible food residue on blade. PIC had item cleaned and sanitized. Working containers of poisonous or toxic materials not properly labeled (critical, corrected during inspection). Spray bottle in shelf of cart in warewashing area was not labeled with common name. PIC had labeled at time of inspection.

Lexington-Shelby Elks Lodge No. 2554, 320 E. Main St., Lexington, June 22. No air gap or approved backflow prevention device on the plumbing system (critical). Observed drain from ice storage bin and drain from designated prep sink to not be air gapped at floor drain. Correct by July 6.

Steve's Dakota Grill, 3101 Park Avenue West, Mansfield, June 23. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Meat slicer had visible residue on blade. PIC had unit cleaned and sanitized. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Scissors used for opening food containers had visible build up. PIC removed from service.

Chlorine sanitizing solution at incorrect temperature and/or concentration. Sanitizer in warewashing machine had less than 10 ppm (critical, corrected during inspection). PIC adjusted so unit was between 50 and 100 ppm. Facility is to regularly monitor unit to ensure sanitizer strength is correct.
