Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between Jan. 25 and Jan. 27:

Avita Health Systems, 715 Richland Mall Drive, Ontario, Jan. 25. Equipment food-contact surfaces or utensils are unclean (critical repeat, corrected during inspection). Observed paddle attachment for large stand mixer and food slicer in clean storage to have visible dried food residues. TCS foods not being cold held at the proper temperature (critical repeat, corrected). Observed multiple TCS foods in cold holding unit (hard boiled eggs, salami, pepperoni, ham, turkey and American cheese) along service line in dining area to be between 44 and 52 degrees. Person in charge (PIC) voluntarily discarded listed foods. Also observed sliced cheddar cheese, pepper jack, provolone, Asian sauce, BBQ sauce, and cut sausage to be at 43 degrees. PIC placed items back in walk in cooler to cool back down to 41 degrees or less.

Applebee's Neighborhood Grill & Bar, 145 Stander Ave., Mansfield, Jan. 25. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed guacamole, sour cream and coleslaw on the make line with internal temperatures of 45-48 degrees. PIC voluntarily discarded items.

Gordon Food Service Store, 1310 N. Lexington Springmill Road, Mansfield, Jan. 25. Food package(s) received in poor condition (critical, corrected during inspection). Observed two cans of pineapple with dents along the seal. PIC removed from sale. TCS foods not being cold held at the proper temperature (critical, corrected). Observed cut melon at 44 degrees in front merchandise cooler. PIC voluntarily discarded three containers of cantaloupe and one package of pineapple. Water source and/or hot water capacity not sufficient (critical). Observed hot water at three-compartment sink to only reach 106 degrees. Ensure hot water is able to reach at least 110 degrees for proper ware-washing. Correct by Jan. 26.

Kroger Company #N836, 1500 Lexington Ave., Mansfield, Jan. 25. Presence of live insects (critical repeat). Observed small black flying insects in the drain areas of the deli. Completely clean the area and contact a licensed pest control provider by re-inspection Feb. 1.

Boardwalk Foods Inc., 98 N. Main St., Mansfield, Jan. 26. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed a bag of diced ham on a shelf below raw sausage in the sliding door cooler in the kitchen. Working containers of poisonous or toxic materials not properly labeled (critical, corrected). Observed a bucket of bleach water in the kitchen that was not labeled.

Athens Greek Restaurant, 41 S. Lexington Springmill Road, Ontario, Jan. 26. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed raw meat on a shelf above boxes of tomatoes in the cooler. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed a container of tzatziki sauce cups in the cooler without a date mark.

Speedway #3624, 2132 W. Fourth St., Mansfield, Jan. 26. Written procedures and plans are not maintained and implemented (critical). No cleaning and maintenance logs observed for f'real machine. Correct by Feb. 2. Equipment food-contact surfaces or utensils are unclean (critical). Observed inner surfaces of ice dispenser on big gulp machine to contain visible residue. Clean by Feb. 2. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical repeat, corrected during inspection). Observed container of hot dogs in walk-in cooler date marked for 11 days total. PIC correct date marking for seven total days from date of prep.

Gwirtz Foods, 5001 Ohio 39, Shelby, Jan. 26. Working containers of poisonous or toxic materials not properly labeled (critical repeat, corrected during inspection). Observed container of smokehouse cleaner to be in the sink without common name on the bottle. Food contact surfaces not easily cleanable (critical, corrected). Observed freezer paper being used on the sandwich station as the food contact portion of the table. Paper was stained and ripped. PIC had new paper placed on table, as a temporary solution until table can be resurfaced.

Olive Garden Italian Restaurant #1390, 558 Lexington Springmill Road, Mansfield, Jan. 26. Presence of live insects (critical repeat). Observed multiple small flying insects in north and south servers stations. At follow up inspection, continued to observe small flying insects in areas around both servers stations, most notably close to ceiling. GM states they are working with supervisor from pest control company on a weekly basis and are utilizing spray provided by pest control company twice daily. Plan is in place to fix grout that is holding water in servers station area and cleaning drains. PIC also states corporate is working with them as well. Less insects observed at re-inspection and areas visibly clean. Re-inspection to take place only if problem persist.

Tom's Tavern, 902 Springmill St., Mansfield, Jan. 26. Handwashing sink is being used for purposes other than handwashing (critical repeat, corrected during inspection). Observed bowl sitting in hand sink.

Volunteers of America Ohio & Indiana, 921 N. Main St., Mansfield, Jan. 27. Quaternary ammonium sanitizing solution at incorrect temperature, concentration and/or water hardness (critical). Observed no quat sanitizer in three-compartment sink.Correct by Jan. 31.

Little Caesar's Pizza #422, 1245 Park Avenue West, Mansfield, Jan. 27. Handwashing sink by the walk-in cooler not accessible (critical repeat). Observed a pallet of bottled drinks in front of the hand sink. Correct by Feb. 3. Quaternary ammonium sanitizing solution at incorrect temperature, concentration and/or water hardness (critical repeat). Observed the quat sanitizer solution dispensing in the three-compartment sink at approximately 100 ppm at the time of inspection. Correct by Feb. 3. Insufficient air gap between the flood rim and the water supply inlet (critical repeat). Observed the spray nozzle above the three-compartment sink hanging below the flood rim of the sink. Correct by Feb. 3.
