Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between June 23 and June 30:

Camp Otyokwah, 3380 Tugend Road, Butler, June 23. Food employee(s) did not wash hands when required (critical, corrected during inspection). Food worker observed donning new gloves without first washing hands.

Panda Express, 994 N. Lexington-Springmill Road, Ontario, June 24. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Container of rice on hot holding unit for rice was uncovered and not in use. Person in charge (PIC) had covered. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection).

Sauce container cart observed in walk-in cooler and not covered. PIC was having items covered at time of inspection. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Single use items observed under drips from wall box by drive-thru. PIC voluntarily discarded affected single use items. PIC is going to have area cleaned prior to storing items there.

Scoops Frozen Treats, 460 Biscayne Road, Mansfield, June 27. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed waffle cones not covered to protect from contamination. PIC placed cover over product. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Observed hot dogs and chicken salad not properly date marked. PIC properly date marked products. Reviewed date marking procedures.

Pleasant Hill Golf Course, 4089 Pleasant Hill Road, Perrysville, June 29. Food employee(s) did not wash hands when required (critical, corrected during inspection). Food employee donned new gloves without first properly washing hands. Food equipment surfaces not cleaned at required frequency (critical, corrected during inspection).

Probe thermometer was not sanitized before taking temperature of hot dog. Corrected; thermometer was sanitized with swab. Please sanitize before and after food contact surfaces. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Half gallon of milk observed open in cooler for over 24 hours without proper date marking. Milk was date marked to June 23.

Subway #119, Ontario, 1330 N. Lexington Springmill Road, Mansfield, June 29. TCS foods not being cold held at the proper temperature (critical repeat, corrected during inspection). Observed TCS foods along make line (35 ounces of chicken, 47-50 degrees; 25 ounces of beef, 50-51 degrees; 8 ounces of cappicola, 47 degrees; and five ounces of bologna, 47-50 degrees) to be above 41 degrees. PIC discarded at time of inspection.

Also observed multiple items (chicken, provolone cheese, steak) inside extra cooler behind front counter to be between 43-45 degrees. PIC states those were just placed in the cooler recently and placed them back in the walk-in cooler to bring temps back down to 41 degrees. PIC also called in a service request at time of inspection for both units.

M and S Drive Thru, 753 Springmill St., Mansfield, June 29. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Storage containers observed in clean area stored with food contact surface in contact with other containers with visible residue. PIC moved to three-compartment sink to rewash, rinse and sanitize. TCS foods not being cold held at the proper temperature (critical, corrected during inspection).

Chicken wings in two-door cooler had internal temperatures of 48-50 degrees for an undetermined amount of time. PIC voluntarily discarded. Improper display of poisonous or toxic materials offered for retail sale (critical, corrected during inspection). Chemicals stored on shelf above soda in retail area. PIC moved at time of inspection.

Kroger Company #N836, 1500 Lexington Ave., Mansfield, June 30. No air gap or approved backflow prevention device on the plumbing system (critical, corrected during inspection). Bulk water machine was not equipped with internal backflow and external was not located at time of inspection. PIC removed from service until corrected.

Little Apple Golf Course, 366 Main St., Bellville, June 30. Food equipment surfaces not cleaned at required frequency (critical, corrected during inspection). Probe thermometer food contact surface was not sanitized before use. Thermometer was swabbed before and after with alcohol swab.
