Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between Aug. 27 and Aug. 31:

Country Meadow Rehabilitation & Nursing Center, 4910 Algire Road, Bellville, Aug. 29. Handwashing sink is being used for purposes other than handwashing (critical, corrected during inspection). Handwashing sink was observed used for draining sanitizer from wiping cloth buckets. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Vitamin D Milk observed stored in cooler for over 24 hours without proper datemarking. Listed item datemarked to Aug. 28.

Pretzelmaker (Richland Mall), 2208 Richland Mall, Space #c-8, Mansfield, Aug. 29. Improper reheating of food for hot holding (critical repeat, corrected during inspection). Observed pre-portioned 2 ounce cups of previously heated and cooled cream cheese to only be between 122 and 130 degrees in hot holding case. Ambient air in hot holding unit observed at 146 degrees.

Employee states cream cheese is placed directly in hold holding case from cooler to reheat for serving. Cream cheese had been in case for an hour per employee. Employee placed cups in pan on steam warmer to bring to reheating temperature of 165 degrees during inspection. At two hours, observed 10, 2 ounce cups of cream cheese to still only be 146-155 degrees. Employee voluntarily discarded these cups during inspection.

Tamara K. King, 4218 Park Avenue West, Mansfield, Aug. 29. Handwashing sink not accessible (critical, corrected during inspection). Observed employee's personal belonging sitting in hand sink at time of inspection. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical, corrected during inspection). Observed working bottle of chlorine sanitizer in bar area to be at 10 ppm. Employee discarded sanitizer during inspection.

Arbors At Mifflin, 1600 Crider Road, Mansfield, Aug. 29. Food contact surfaces not easily cleanable (critical, corrected during inspection). Observed can-opener blade with residue on blade.

Deer Ridge Golf Acquisition LLC, 900 Comfort Drive, Bellville, Aug. 30. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical repeat, corrected during inspection). Observed employee illness agreement that was incomplete and only contained five reportable illness of form. RPH food employee illness reporting agreement provided and person in charge (PIC) had kitchen staff sign during inspection. Food employee(s) did not wash hands when required (critical, corrected during inspection). Observed food employee handle raw meat with gloved hands followed by opening refrigerator and grabbing pre-packaged ready-to-eat food without changing gloves and washing hands.

Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed multiple food items in various locations throughout kitchen to not be covered/protected: Croutons on soup counter,, baked potatoes in refrigerator, mushrooms on sheet pan, hard boiled egg in salad cooler and pickles in refrigerator. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed can opener to contain visible residue. Manual wash solution not being maintained at 110 degrees or above (critical, corrected during inspection). Observed wash solution in three-compartment sink to be at 108 degrees. PIC added hot water and brought temperature up to 116 degrees during inspection.

V & M Restaurant, 104 Main St., Bellville, Aug. 30. PIC unable to demonstrate knowledge of cleaning and sanitizing (critical). Test stips were not available and PIC did not know the required concentration for sanitizer in wiping cloth buckets. Ensure quaternary ammonia sanitizer is kept at concentration as prescribed on directions. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected during inspection). Ice used for beverages observed with lid of machine open near hand wash station.

Single-use gloves used improperly (critical, corrected during inspection). Female worker observed touching RTE tomatoes and RTE lettuce with gloved hands, putting them in salad, walking to cooler and touching contaminated surface, the cooler door, then touching ready-to-eat vegetables with same gloved hands. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical repeat, corrected during inspection). Gallon of dairy milk observed stored in display cooler by pies for over 24 hours without proper datemarking. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical repeat, corrected during inspection). Gallon of dairy milk in Berg cooler observed stored for over 24 hours without proper datemarking.

Presence of live insects, rodents, and other pests (critical). Several flying insects observed in food prep area. Correct by Sept. 13. Food contact surfaces not easily cleanable (critical, corrected during inspection). Two spatulas observed not smooth and cracked. PIC voluntarily discarded listed equipment.

Ducky's Pub And Grill, 9 Cleveland St., Butler, Aug. 31. Ready-to-eat, TCS food not properly discarded when required (critical, corrected during inspection). Bag of lettuce observed in pizza room food prep cooler that was supposed to be discarded.
