Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between Jan. 10 and Jan. 13:

Tim Hortons, 1072 Ashland Road, Mansfield, Jan. 10. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected during inspection). Sani bucket was 100 ppm. Employee dumped and made a new sani bucket with correct concentration.

Old Carolina BBQ Company, 2035 W. Fourth St., Mansfield, Jan. 10. TCS foods not being hot held at the proper temperature (critical repeat, corrected during inspection). BBQ ribs in hot holding at 125 degrees. Person in charge (PIC) stated it had been in hot holding less than 60 minutes. PIC put ribs back into smoker and reheated to 165 degrees.

Wendy's, 1372 Lexington Ave., Mansfield, Jan. 10. PIC did not ensure employees are properly maintaining temperatures of TCS foods during hot and cold storage (critical repeat, corrected during inspection). Dining room make table upper area had an internal temperatures above 41 degrees. PIC removed Items. Facility is to monitor cold holding unit before and during use to ensure it is functioning as designed. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Food storage containers in clean area had visible residue. PIC had items moved to warewashing to clean.

TCS foods not being cold held at the proper temperature (critical repeat, corrected during inspection). Dining side make table top items had internal temperatures of 48-51 degrees. PIC voluntarily discarded TCS items. Improper use of time as a public health control (6 hours) (critical, corrected during inspection). Cheese was not time stamped with time placed out on line. Improper storage of poisonous or toxic materials (critical, corrected during inspection). Hand sanitizer stored on shelf above raw bacon.

Roosters Mansfield, 2140 W. Fourth St., Mansfield, Jan. 11. Handwashing sink is being used for purposes other than handwashing (critical, corrected during inspection). Observed a red sanitizer bucket in the handwashing sink on the make line. TCS foods not being hot held at the proper temperature (critical, corrected during inspection). Observed recently prepared mac n cheese at 127 degrees. PIC had mac n cheese reheated to hold above 135 degrees at the time of inspection. Manual wash solution not being maintained at 110 degrees or above (critical, corrected during inspection). Observed wash water at 105 degrees at the three compartment sink in the bar.

Subway #12061, 950 Ashland Road, Mansfield, Jan. 12. Insufficient air gap between the flood rim and the water supply inlet (critical, corrected during inspection). Observed a ring around the end of the drain pipe underneath the prep sink that was eliminating the air gap. PIC adjusted ring to create at least a one inch air gap between the end of the drain pipe and the floor drain under the prep sink at the time of inspection.

Cracker Barrel Old Country Store Inc #224, 100 Stander Ave., Mansfield, Jan. 12. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Food in freezer and cooler at the cook line was not covered.

Kentucky Fried Chicken, 1447 Lexington Ave., Mansfield, Jan. 12. PIC not ensuring employees are properly sanitizing equipment and utensils and monitoring of sanitizer parameters (critical, corrected during inspection). Sanitizer strength in three compartment sink and sanitizer buckets was 0 PPM. PIC had new bag installed and sanitizer remade. IC unable to demonstrate knowledge of temperatures and times for safe refrigerated food storage, hot holding, cooling and reheating (critical, corrected during inspection). PIC unable to state the times and temperatures needed for cooling of foods. Sanitarian provided handout at time of inspection.

Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical repeat, corrected during inspection). Facility could not locate where employees had been informed in a verifiable manner of there responsibility to report. Sanitarian provided illness agreement at time of inspection. Handwashing sink not accessible (critical, corrected during inspection). Chemical bottle observed stored in hand sink by back door. Handwashing sink is being used for purposes other than handwashing (critical, corrected during inspection).

Food debris and waste observed in hand sink by drive through. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected during inspection). Warewashing three compartment sink and sanitizer buckets had strength of 0 ppm.

Hesham Quick Stop Inc., 398 S. Diamond St., Mansfield, Jan. 13. No person in charge present in food facility during inspection (critical).

Winchester Terrace Nursing Home, 70 Winchester Road, Mansfield, Jan. 13. Handwashing sink is being used for purposes other than handwashing (critical, corrected during inspection). Observed a pitcher sitting in the handwashing sink. Quaternary ammonium sanitizing solution at incorrect temperature, concentration and/or water hardness (critical, corrected during inspection). Observed the sanitizing solution in the three compartment sink at 100 ppm. PIC remade solution to a concentration of 200 ppm.

Ready-to-eat, TCS food not properly discarded when required (critical, corrected during inspection). Observed a container of tuna salad in the walk-in cooler that was prepared more than seven days ago (Jan. 4). PIC voluntarily discarded tuna salad. Manual wash solution not being maintained at 110 degrees or above (critical, corrected during inspection). Observed the wash water in the three compartment sink at 106 degrees. PIC drained and refilled solution.

Paul Revere Inn, 57 Ohio 97, Lexington, Jan. 13. Person in charge not ensuring proper cooling (critical, corrected during inspection). Potatoes in walk-in cooler in deep pan cooling with lid. PIC had moved to thin layer less than one inch thick on sheet and loosely covered. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Containers in freezer by cook line did not have lids. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical, corrected during inspection). Sanitizer bucket in bar area had strength below 10 ppm.

Equipment food-contact surfaces or utensils are unclean (critical repeat, corrected during inspection). Can opener blade had dried-on residue. Improper cooling of TCS food (critical, corrected during inspection). Partially cooked potatoes in walk-in cooler that had been in there approximately 45 minutes had an internal temperature of 98 degrees. PIC had moved to thin sheets and rapidly cooled.

TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Salad make table on one side had internal temperatures of 44 degrees for an undetermined amount of time. PIC voluntarily discarded 3 quarts of cottage cheese and 1 quart of coleslaw. Ambient temperature was at 38 degrees.
