Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between Nov. 12 and Nov. 18:

Mansfield Memorial Homes, 50 Blymyer Ave., Mansfield, Nov. 12. Presence of live insects, rodents, and other pests (critical repeat). Droppings observed in mop room. Person in charge (PIC) has invoice from licensed applicator Nov. 9. Have areas cleaned. Correct by Nov. 15.

Pretzelmaker, 2208 Richland Mall, Space #c-8, Mansfield, Nov. 12. Working containers of poisonous or toxic materials not properly labeled (critical, corrected during inspection). Observed a spray bottle of sanitizer in the restroom without a label. PIC labeled bottle with contents at the time of inspection.

AMVETS Post #26, 1100 W. Fourth St., Mansfield, Nov. 15. Hot water for mechanical sanitization is below required temperature (critical). Final rinse in dishwasher not bringing surface temperature of utensils up to 160 degrees. Plate simulator thermometer reading 154 degrees at the time of inspection. Correct by Nov. 22. Ready-to-eat, TCS food not properly discarded when required (critical repeat, corrected during inspection).

Observed a bag of bologna slices with a date mark of Nov. 1. Ready-to-eat TCS food shall be kept for a maximum of seven days once prepared or opened. PIC voluntarily discarded bologna. Working containers of poisonous or toxic materials not properly labeled (critical, corrected during inspection). Observed a spray bottle of degreaser without a label in the cabinet. PIC labeled bottle with contents.

Kentucky Fried Chicken, 301 Ashland Road, Mansfield, Nov. 15. Manual wash solution not being maintained at 110 degrees or above (critical repeat). Observed wash station reading 93 degrees. Water shall maintain 110 degrees during wash cycle.

Penn Station, 2166 Walker Lake Road, Mansfield, Nov. 15. PIC not ensuring employees are properly sanitizing equipment and utensils and monitoring of sanitizer parameters (critical, corrected during inspection). Employee washing dishes without sanitizing sink set up. Employee emptied sink and set it up properly, rewashing equipment. PIC unable to demonstrate knowledge by having no critical violations (critical). PIC not ensuring employees are properly sanitizing equipment and utensils and monitoring of sanitizer parameters (critical, corrected during inspection). PIC did not sanitize probe before taking a temperature. Had contaminated French fries voluntarily destroyed.

Food employee(s) did not wash hands when required (critical repeat, corrected during inspection). Employee drank from a cup, checked his phone and then went to go back to work, putting on gloves without washing hands first. Had employee remove and dispose of gloves and wash hands before putting on clean gloves. Handwashing sink is being used for purposes other than handwashing (critical repeat, corrected during inspection). Observed hand sink with metal dish scrubber, sanitizer bucket and used dish towel in it. PIC removed items. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Raw shell eggs stored on top of a box of green peppers. PIC moved eggs.

Food not properly protected from contamination by separation, packaging and segregation (critical repeat). Sliced lemons in a container in the dining room did not have a lid to protect them. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Sliced tomatoes and cut lettuce in the make table top at 49 degrees. Tomatoes were not cooled to 41 degrees before placing them in the make table top. Five pounds of lettuce and 3 pounds of sliced tomatoes were voluntarily destroyed.

Improper use of time as a public health control - 4 hours (critical repeat, corrected during inspection). French fries being held as time without temperature were not time stamped. Food thermometers are not accurate (critical). Thermometer used for French fries was reading 23 degrees when actually the temperature was 29 degrees. Thermometer shall be accurate to a plus or minus of 2 degrees.

Lexington Central Elementary, 124 Frederick St., Lexington, Nov. 16. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Almond milk observed opened in cooler for over 24 hours without proper date marking. Corrected: marked with Nov. 21 date.

Speedway #3624, 2132 W. Fourth St., Mansfield. Nov. 17. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical repeat). There is a written policy, but it has not been signed. Food not properly protected from contamination by separation, packaging and segregation (critical). Apples sold self service are not washed and wrapped. PIC will tell vender the apples must be washed and wrapped. Correct by next inspection.

Whit's Frozen Custard, 1531 Lexington Ave., Mansfield, Nov. 17. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Cakes in walk-in freezer were uncovered. Improper reheating of food for hot holding (critical, corrected during inspection). Shredded chicken placed in hot holding unit set for 140 degrees. Items need to be reheated to 165 degrees prior to hot holding at 135 degrees or above. PIC had reheated in microwave.

Mansfield Christian Elementary School, 500 Logan Road, Mansfield, Nov. 17. TCS foods not being hot held at the proper temperature (critical, corrected during inspection). Hamburgers on serving line were measured at 121 degrees. All hamburgers taken off serving line and reheated to 165 degrees.

Circle K #4705374, 2094 Park Avenue West, Mansfield, Nov. 17. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Whole milk and creamers inside the dispenser cold holding between 52-56 degrees. PIC voluntarily discarded milk and creamers at the time of inspection. Missing and/or incomplete allergen declaration(s) (critical). List of ingredients for chocolate chip cookies inside the retail display case not available for customer review. Correct by Dec. 1.

Taco Bell # 30674, 1194 Park Ave. W., Mansfield, Nov. 18. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected during inspection). Sanitizer solution in three compartment sink had a temperature of 122 degrees and a strength of over 400 PPM. Direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed (critical, corrected during inspection). Drain from soda dispenser in drive thru area was below flood rim of floor drain.

Burger King Restaurant # 415, 560 N. Lexington Springmill Road, Mansfield, Nov. 18. Improper use of time as a public health control - 4 hours (critical, corrected during inspection). Observed a container of sliced ham on the make line at room temperature without a time stamp to mark the four hour discard time. PIC added time sticker with discard time.
