Richland County restaurant inspections: critical violations
The following restaurant inspections with critical violations were conducted by Richland Public Health between March 18 and March 23:
• Butler Elementary School, 125 College St., Butler, March 18. Person in charge (PIC) unable to demonstrate knowledge by having no critical violations (critical, corrected during inspection). Ready-to-eat vegetables were in contact with nonfood contact surface of packaged carrots in same bowl. Raw ready-to-eat cucumbers and broccoli were washed and put in separate bowls from carrots.
Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Prepackaged carrots with nonfood contact surface of packages were in same bowl with raw ready-to-eat cucumbers and broccoli. Carrots were segregated from broccoli and cucumbers and put in separate bowls.
• Madison Shell, 899 Ashland Road, Mansfield, March 18. Water under pressure is not available to all fixtures as required (critical). Observed low hot water at hand wash station. Correct ASAP.
• Big Lots #5220, 929 N. Lexington Springmill Road, Ontario, March 18. Food package(s) received in poor condition (critical, corrected during inspection). Observed two cans of tomato soup and one can of pinto beans with a dent on the seal. PIC voluntarily discarded products.
• Bell Stores #112, 1330 N. Lexington Springmill Road, Mansfield, March 18. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). Employee was unable to locate signed copies of the employee health policy. Employee had a blank copy to have employees read and sign. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Observed open packages of hot dogs, sausages and taquitos not dated. PIC verified open date and properly date marked.
Improper display of poisonous or toxic materials offered for retail sale (critical, corrected during inspection). Observed eye drops stored above beverages on sale floor. PIC pulled to be relocated, separated from food items. Food display not properly protected from contamination by consumers (critical, corrected during inspection). Observed apples in customer self-service area not protected from contamination. Employee pulled from sale area. Equipment and/or utensils improper construction (critical, corrected during inspection). Observed coffee beans stored directly in a non-food grade container. Employee voluntarily discarded container and product.
• The Golden Burrito #2, 1740 Park Avenue West, Ontario, March 21. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed can opener to have food residue and buildup. PIC removed from prep table and will purchase a new one. Correct by operation.
Insufficient air gap between the flood rim and the water supply inlet (critical, corrected during inspection). Observed prep sink to not have air gap between spray head and top of sink basin. PIC able to adjust spring on spray head to allow for sufficient air gap; plans to remove sprayer from prep sink.
• Yana Mini Mart Inc., 98 Lexington Ave., Mansfield, March 21. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). PIC unable to locate signed copies of employee health policies. Provided RPH's employee illness reporting agreement form; PIC signed and posted. Food not properly protected from contamination by separation, packaging, and segregation (critical repeat, corrected during inspection). Observed raw bacon on a shelf above canned drinks inside of the walk-in cooler. PIC moved bacon to the bottom shelf inside the cooler.
Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed cartons of raw shell eggs stored on the shelf above ready-to-eat foods inside the reach-in sandwich cooler. PIC moved eggs to the bottom shelf. Improper storage of poisonous or toxic materials (critical, corrected during inspection). Observed gallons of windshield washer fluid stored on a shelf above canned drinks in the back storage area. PIC moved windshield washer fluid to the bottom shelf below drinks. Missing and/or incomplete allergen declaration(s) (critical repeat, corrected during inspection).
Observed sleeves of Ritz and saltine crackers for sale without a list of ingredients, common name, net weight or address of packaging facility on the package. PIC removed crackers from sale. Food in an RFE must be embargoed due to adulteration, misbranding, or baby food or formula is expired (critical, corrected during inspection). Observed two 12.6 ounce cans of Nutramigen formula with a use by date of Feb. 1. PIC voluntarily discarded formula.
• The Richland School Of Academic Arts, 1456 Park Avenue West, Mansfield, March 21. Working containers of poisonous or toxic materials not properly labeled. Sanitizer solution in kitchen was not labeled with common name (critical, corrected during inspection).
• Aldi, Inc #48, 955 N. Lexington Springmill Road, Ontario, March 22. Food not properly protected from contamination by separation, packaging, and segregation (critical, corrected during inspection). Observed raw ground turkey and pork sausage stored over ready-to-eat (RTE) wine.
• Liberty Nursing Center of Mansfield, 535 Lexington Ave., Mansfield, March 22. PIC not ensuring employees are properly sanitizing equipment and utensils and monitoring of sanitizer parameters (critical, corrected during inspection). Observed warewashing machine to not have proper sanitizing solution strength. PIC had employees shift to sanitizing via manual warewashing. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). Employee present at time of inspection did not have employee health agreement on file. PIC had employee read and sign agreement.
Chlorine sanitizing solution at incorrect temperature and/or concentration (critical, corrected during inspection). Observed warewashing machine sanitizing solution concentration tested to be below 10 ppm. Facility is no longer using mechanical warewashing unit until it can be verified that unit is able to sanitize at proper strength. Equipment food-contact surfaces or utensils are unclean (critical repeat, corrected during inspection). Observed visible black residue along edges of ice production area inside bulk ice machine. Facility discontinued use until proper cleaning and sanitization can take place.
Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Observed gallon of milk that had been opened for longer than 24 hours and in walk-in cooler to not have date marking. No air gap or approved backflow prevention device on the plumbing system (critical, corrected during inspection). Observed bulk ice machine to not have air gap at flood level rim of drain. Facility removed bulk ice machine from service until air gap can be provided.
• National Parkway Duke/Duchess/BP1186, 1020 National Parkway, Mansfield, March 23. Working containers of poisonous or toxic materials not properly labeled (critical, corrected during inspection). Spray bottle of sanitizer in kitchen was not labeled with common name.
