Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between Sept. 21 and Sept. 25:

3rd Cup Tea, 101 N. Main St., Mansfield, Sept. 21. Water source and/or hot water capacity not sufficient (critical). Hot water was not consistent. Provide required hot water during all hours of operation.

98 Lex Food Mart, 98 Lexington Ave., Mansfield, Sept. 21. Food package(s) received in poor condition (critical, corrected during inspection). Observed a can of mixed fruit with dents on the top and bottom seams. Person in charge (PIC) removed fruit can from sale. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Observed a carton of raw eggs on a shelf above ready-to-eat foods inside the reach-in display cooler.

Park Avenue Patio LLC, 1349 Park Avenue West, Mansfield, Sept. 21. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical repeat, corrected during inspection). PIC unable to locate signed copies of employee illness reporting agreements. Provided RPH's employee illness reporting agreement. Working containers of poisonous or toxic materials not properly labeled (critical repeat, corrected). Observed a spray bottle of degreaser that was not labeled.

Tim Hortons, 1072 Ashland Road, Mansfield, Sept. 21. Hand washing sink not accessible (critical, corrected during inspection). Observed containers and hoodie blocking hand sink access.

Subway Sandwich Shoppe #38123, 359 N. Lexington Springmill Road, Ontario, Sept. 21. Presence of live insects (critical repeat). At re-inspection continued to observe multiple flying insects that appear to be gnats or drain flies, as well as large flies throughout the facility. Observed them in the back food prep area, front make line area and around fountain beverage station. Facility needs to contact licensed pest control company and request reports to maintain records of any visits/treatments. Correct by Sept. 28.

Lex Lanes, Lex-Luray Enterprises Inc., 60 Ohio 97, Lexington, Sept. 21. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed residue on the underside of the soda nozzle in the bar.

German Home Society Inc./ Sons of Herman, 416 Orange St., Mansfield, Sept. 21. TCS foods not being cold held at the proper temperature (critical). Observed Magali stand-up cooler that read 49 degrees. Ask PIC to remove perishable food ASAP. Correct by Sept. 25.

Malabar Intermediate School, 205 W. Cook Road, Mansfield, Sept. 21. Unsafe, adulterated or not honestly presented food not discarded or properly reconditioned (critical, corrected during inspection). Damaged can of mandarin oranges observed stored in dry goods room. Listed item was set aside for inventory and scheduled to be discarded.

Dollar General #21644, 946 Springmill Road, Mansfield, Sept. 22. Person in charge did not ensure that employees observed foods as they were received (critical, corrected during inspection). PIC stated the facility does not take temperatures upon receipt of shipments. Education was provided. Food package(s) received in poor condition (critical repeat, corrected). Observed one can of refried beans to have sharp dents. PIC voluntarily discarded can.

J B's Drive In, 265 S. Delaware St., Lexington, Sept. 22. TCS foods not being hot held at the proper temperature (critical, corrected during inspection). Observed shredded chicken in the warmer at 121 degrees. PIC adjusted warmer to bring temperatures above 135 degrees. Food thermometers are not accurate (critical, corrected). Observed the dial stem probe thermometer in the shredded chicken reading 130 degrees. Internal temperature of chicken was 121 degrees. PIC calibrated thermometer. Food contact surfaces not easily cleanable (critical, corrected). Observed a cracked plastic pitcher. PIC voluntarily discarded the pitcher.

Springmill Corner Market, 939 Springmill St., Mansfield, Sept. 22. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Observed pizza subs date marked with an eight-day date marking system; also found turkey and American, roast beef and hot pepper, and ham and swiss subs with a 10 day date marking system. Ensure foods are date marked with a seven day (six days plus the date of opening) date marking system.

Ontario Mansfield Y Child Care, 2200 Bedford Blvd., Mansfield, Sept. 25. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed TCS foods (one gallon of milk, 40 sticks of string cheese, 28 yogurt containers) in refrigerator in infant room held above 41 degrees for an undetermined amount of time. All listed foods were discarded. PIC adjusted thermostat on refrigerator. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed open gallon of whole milk in refrigerator in infant room not date marked. Milk was discarded. Richland Public Health (RPH) date marking educational handout provided.

Kids Korner Preschool & Child Care, 124 Plymouth St., Bldg. C, Lexington, Sept. 25. Person in charge did not ensure that employees observed foods as they were received (critical, corrected during inspection). PIC stated that temperatures are not being taken to ensure received foods come in at proper temperature. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed no date mark on the milk. PIC verified the milk had been opened on Sept. 20.

Dollar General Store #14503, 458 Springmill St., Mansfield, Sept. 25. Food package(s) received in poor condition (critical, corrected during inspection). Observed one can of cream corn, a three-pack of albacore tuna, one can of beef stew, one can of chicken and dumplings, and one can of chicken meatball Spaghettios to be dented on the seams or to the extent where the quality of the contents cannot be guaranteed. PIC voluntarily discarded.

Ollie's Bargain Outlet #49, 1414 Lexington Ave., Mansfield, Sept. 25. Food package(s) received in poor condition (critical repeat, corrected during inspection). Observed one can of garbanzo beans and one can of peeled tomatoes to be dented on the side seam. PIC had cans removed from sale.
