Richland Public Health restaurant inspections for Jan. 17-23, 2024

The following restaurant inspections with critical violations were conducted by Richland Public Health between Jan. 17 and Jan. 23:

Richland Correctional Institution, 1001 Olivesburg Road, Mansfield, Jan. 17. Improper washing of hands and arms (critical, corrected during inspection). Food worker observed touching contaminated surface with cleaned hands while washing. Working containers of poisonous or toxic materials not properly labeled (critical, corrected). Sanitizer bucket observed without proper labelling. Food contact surfaces not easily cleanable (critical repeat, corrected). Cooking pans observed dented and not easily cleanable. Person in charge (PIC) has ordered new cooking pans.

Lorain Music/Longview Steel, Lorain Music & Vending Co. Inc., 1555 W. Longview Ave., Mansfield, Jan. 17. Ready-to-eat, TCS food not properly discarded when required (critical, corrected during inspection). Four sandwiches date marked Jan. 15 observed in machine. Grilled ham, beef charbroil and two cheeseburgers were voluntarily discarded.

Fairfield By Marriott, 1065 N. Lexington Springmill Road, Ontario, Jan. 19. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). PIC unable to locate employee illness agreements. Copy of RPH illness reporting agreement provided and employee read and signed during inspection. TCS foods not being cold held at the proper temperature (critical, corrected). Observed tray of boiled eggs and container of salsa in reach-in cooler held above 41 degrees for undetermined amount of time. PIC voluntarily discarded listed items.

Wingate By Wyndham, 2069 Walker Lake Road, Ontario, Jan. 19. No air gap or approved backflow prevention device on the plumbing system (critical repeat). No backflow device observed on the water supply for the Bunn coffee maker in the kitchen area. Correct by Feb. 2.

Pizza Hut, 365 N. Lexington Springmill Road, Mansfield, Jan. 19. PIC unable to demonstrate knowledge by having no critical violations (critical repeat). Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected during inspection). Observed oil to be uncovered in the prep area. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed fryer baskets to have visible residue. Equipment food-contact surfaces or utensils are unclean (critical repeat, corrected). Observed food contact portions of pans and bowls stacked in clean dish areas to have both sticker residue and food residue. Insufficient air gap between the flood rim and the water supply inlet (critical repeat). Observed no air gap under the cabinet near the server station soda dispenser. Correct by re-inspection Jan. 26.

Winchester Terrace Nursing Home, 70 Winchester Road, Mansifeld, Jan. 19. Manual wash solution not being maintained at 110 degrees or above (critical repeat, corrected during inspection). Observed the wash water temperature in the three-compartment sink at 91 degrees.

Romeo's Pizza, 523 S. Main St., Mansfield, Jan. 22. TCS foods not being hot held at the proper temperature (critical, corrected during inspection). Observed pizza sauce being held at 116 degrees.Food items shall be maintained at 135 degrees.

Walmart #1539, 359 N. Lexington Springmill Road, Mansfield, Jan. 22. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected during inspection). Observed packages of raw pork jowls and raw salt pork above packages of ready-to-eat turkey drumsticks in the retail meat cooler.

Beautiful Foods LLC, 1393 Lexington Ave., Mansfield, Jan. 22. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical repeat, corrected during inspection). Observed chlorine sanitizer solution concentration above 200 ppm. Missing and/or incomplete allergen declaration(s) (critical repeat, corrected). Observed missing allergen statement information (sulfur dioxides) on the dried apricots, dried pineapple and dried papaya chunks.

Fazoli's, 1048 Lexington Springmill Road, Mansfield, Jan. 22. PIC unable to demonstrate knowledge by having no critical violations (critical). Correct by Jan. 29. Quaternary ammonium sanitizing solution at incorrect concentration (critical, corrected during inspection). Observed multiple working buckets of quaternary ammonium sanitizer throughout kitchen to be under strength. TCS foods not being cold held at the proper temperature (critical repeat, corrected). Observed strawberry puree and bowl of salad/cut lettuce in front counter display cooler to be held above 41 degrees for an undetermined amount of time. PIC voluntarily discarded puree and lettuce. Improper use of time as a public health control ― 4 hours (critical, corrected). Observed time in lieu of temperature policy on file for cooked pasta on make line and for par-baked pizzas. No time stamps observed to indicate 4-hour hold time for cooked fettuccini and spaghetti. PIC discarded fettuccini and time stamped spaghetti. Insufficient air gap between the flood rim and the water supply inlet (critical). Observed sprayer head on three-compartment sink to hang below the flood level rim of the sink. Correct by Jan. 29.

Tom's Tavern, 902 Springmill St., Mansfield, Jan. 22. Handwashing sink not accessible (critical, corrected during inspection). Observed a bowl in the handwashing sink.

Meijer #126, 1355 N. Lexington Springmill Road, Ontario, Jan. 23. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected during inspection). Observed packages of raw chicken bacon next to packages of fully cooked turkey bacon and packages of raw turkey bacon next to packages of raw pork bacon in the retail display cooler.

Planktown Country Market LLC, 1921 Free Road, Shiloh, Jan. 23. PIC did not ensure employees are properly maintaining temperatures of TCS foods during hot and cold storage (critical, corrected during inspection). Observed meat and cheese trims to be sitting on a counter with no employees actively working on the task. Also observed cheddar cheese to be out for extended period of time in the café. PIC unable to demonstrate knowledge of temperatures and times for safe refrigerated food storage, hot holding, cooling and reheating (critical, corrected). PIC stated incorrect time and temperature parameters for cooling of biscuit gravy. PIC unable to demonstrate knowledge by having no critical violations (critical repeat, corrected). Person in charge did not ensure that employees observed foods as they were received (critical repeat, corrected). PIC stated the facility does not take temperature of foods upon receipt. Handwashing sink is being used for purposes other than handwashing (critical, corrected). Observed bread hook and measuring cups in the handwashing sink in the café area. Chemical sanitizer other than chlorine, iodine or a quaternary ammonium applied improperly (critical, corrected). Observed diethyl methyl chloride being used in the deli three-compartment sink at improper concentration. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed shake machine spinner in the café area to have visible residue. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed dirty utensil stored in the clean area in the bakery area. Quaternary ammonium sanitizing solution at incorrect temperature, concentration and/or water hardness (critical repeat, corrected). Observed quaternary ammonium sanitizer bucket in the café area to be below 150 ppm. Improper cooling of TCS food (critical, corrected). Observed ham salad being cooled in big bowl filled to top, rendering the food unable to cool properly. TCS foods not being cold held at the proper temperature (critical repeat, corrected). Observed foods stored in the reach-in café cooler and pie cooler to be between 45-55 degrees. PIC voluntarily discarded all affected product. TCS foods not being cold held at the proper temperature (critical, corrected). Observed trims in the deli to be sitting on a counter at 47 degrees. Working containers of poisonous or toxic materials not properly labeled (critical, corrected). Observed a bucket of soap to be unlabeled. Presence of rodents (critical, corrected). Observed rodent droppings in the dry storage room behind the door. Food contact surfaces not easily cleanable (critical, corrected). Observed pair of tongs in the bakery area that's food contact surface was completely rusted, rendering the tongs no longer smooth, durable, non-absorbent or easily cleanable. PIC discarded tongs.

Cuyahoga Vending Services/Newman Tubing, 100 Cairns Road, Mansfield, Jan. 23. Ready-to-eat, TCS food not properly discarded when required (critical, corrected during inspection). Three sandwiches were observed date marked to be discarded after Jan. 22. Sandwiches were voluntarily discarded.

Cuyahoga Vending Services/Hi-Stat-South Basement, 345 S Mill St., Lexington, Jan. 23. Ready-to-eat, TCS food not properly discarded when required (critical, corrected during inspection). Cheeseburger sandwich observed with no date marking. Listed item was voluntarily discarded.

Olive Garden Italian Restaurant #1390, 558 Lexington Springmill Road, Mansfield, Jan. 23. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed appetizers on tray in walk-in freezer not covered/protected from potential contamination. Quaternary ammonium sanitizing solution at incorrect temperature, concentration and/or water hardness (critical, corrected). Observed working buckets of sanitizer at 0 ppm. Insufficient air gap between the flood rim and the water supply inlet (critical). No air gap observed at flood level rim of floor drain and drain hose from ice storage bin in south servers area. Correct by Jan. 30.

Gionino's Pizzeria, 1289 Ashland Road, Mansfield, Jan. 23. Food contact surfaces not easily cleanable (critical, corrected during inspection). Observed crack lid on chicken breading container. PIC removed lid from service.
