Wood County inspections: Four restaurants receive five or more violations

The following are the latest results of Wood County Health Department inspections on licensed food establishments and other types of businesses, which USA TODAY NETWORK-Wisconsin reports regularly.

Arpin

  • Dupe's Bar and Grill, 6328 Main St.: Jan. 17: One violation: No certified food manager for establishment.

Auburndale

  • Wonder Inn Willy's, 5778 Main St.: Jan. 26: One violation: Scented bleach sanitizer used in establishment not approved.

Marshfield

  • Burger King No. 5278, 1015 N. Central Ave.: Jan. 20: No violations.

  • The Kitchen Table, 118 Third St.: Jan. 3: Two violations: Shelf below hot food unit in kitchen rusty; bathroom doors not kept shut.

  • Little Caesar's Pizza, 408 N. Central Ave.: Jan. 10: One violation: No sanitizer concentration test strip available.

  • Melody Gardens, 1200 S. Oak St.: Jan. 17: Three violations: Sanitizer bucket stored in hand sink in front area; shelves in walk-in cooler rusty.

  • The Neighborhood Bar, 600 W. Blodgett Ave.: Jan. 12: No violations.

  • Southway Pub and Grill, 1212 S. Central Ave.: Jan. 12: No violations.

  • Swamp Bar, 511 S. Central Ave.: Jan. 12: One violation: No approved food protection manager's certificate posted in establishment.

  • T-Bones Bar and Grill, 400 N. Central Ave.: Jan. 12: Four violations: Ice scoop stored with handle down (corrected during inspection); stand-up freezer missing handle; no handwashing sign in men's room (corrected during inspection); bathroom doors not kept closed (corrected during inspection).

Nekoosa

  • Nekoosa Cafe, 324 Market St.: Jan. 18: Two violations: Whipped butter stored at improper temperature (corrected during inspection); kitchen floor tile cracked.

Wisconsin Rapids

  • Aaron's Wines and Steins, 160 First St. N.: Jan. 19: Three violations: Thermometer not functioning; water in ware washing machine not reaching required temperature; irreversible registering thermometer not provided for measuring surface temperature in ware washing machine.

  • The Buck Rub of Wisconsin Rapids, 1511 First St. N.: Jan. 13: One violation: Sanitizer concentration test kit not available.

  • Checkers, 1840 Eighth St. S.: Jan. 27: One violation: Cardboard used to line shelf under flat-top grill.

  • Domino's Pizza, 2880 Eighth St. S.: Jan. 13: One violation: Sanitizer in bucket not used according to label instructions.

  • Econolodge Breakfast, 3120 Eighth St. S.: Jan. 25: One violation: Test kit not available for checking sanitizer.

  • 1st Street Pub, 1451 First St. N.: Jan. 13: No violations.

  • Four Stools Short, 180 Second St. N.: Jan. 27: Two violations: Test kit not available for checking sanitizer; woman's bathroom not provided with covered garbage can.

  • Hollyrocks, 114 Second St. N.: Jan. 6: One violation: No sanitizer concentration test kit available.

  • Johnny's, 2610 Eighth St. S.: Jan 13: One violation: No handwashing sign at men's bathroom sink (corrected during inspection).

  • Monkeys Tavern, 2963 State 73 N.: Jan. 18: No violations.

  • Olympic II Family Restaurant, 2520 Eighth St. S.: Jan. 13: Six violations: Employees not washing hands after cracking eggs and then plating food; employee seen handling read-to-eat food with bare hands; margarine, potatoes and coconut cream pie held at improper temperatures; racks in cooler and dry storage area rusty; exterior of flat-top grill and fryers not cleaned frequently; wall around dish washing area soiled; re-inspected Jan. 14: First three violations corrected; last three violations still exist.

  • Perkins Restaurant and Bakery, 2311 Eighth St. S.: Jan. 12: Seven violations: diced tomatoes, diced ham and sausage in reach-in cooler not at proper temperature; sanitizer not used according to label instructions; spices and dressings in unlabeled containers; ice accumulation in walk-in freezer and it is not in good repair; cutting board on cook line scored; exterior of flat-top grill and fryers not cleaned frequently; ceiling vents throughout kitchen and wall behind ice machine dusty; Re-inspected Jan. 19: First three items corrected; last four items still present.

  • Pizza Hut, 2001 Eighth St. S.: Jan. 6: One violation: Wall behind dish machine and pizza making line soiled.

  • Portesi's Fine Foods, 4110 Eighth St. S.: Jan. 27: No violations.

  • The Ridges, 2311 Griffith Ave.: Jan. 6: Two violations: Waitress cooler unplugged and at wrong temperature (corrected during inspection); sanitizer not used according to label instructions (corrected during inspection).

  • Sportsman's Pub and Grill, 4810 Eighth St. S.: Jan. 13: Three violations: Food in cooler improperly date marked; wiping cloths stored on counter tops; upright freezer missing handle.

  • Square Bar, 210 Johnson St.: Jan. 13: One violation: Bathroom doors not kept closed.

  • Subway No. 131, 3810 Eighth St. S.: Jan. 13: One violation: Chicken not held at proper temperature.

  • Tamarack's Pub, 481 Ruby Ave.: Jan. 13: Five violations: Au jus in refrigerator not at proper temperature within six hours (corrected during inspection); ground beef improperly thawed (corrected during inspection); dressing stored in unlabeled containers; shelves in walk-in cooler rusty; flooring in kitchen cracked.

  • Walmart Subway, 4331 Eighth St. S.: Jan. 6: No violations.

  • The Whitney, 451 E. Grand Ave.: Jan 25: Six violations: Consumer advisory missing disclosure statement; ice accumulation in ice cream freezer; cover for condenser on ice machine missing and not in good repair; salamander broilernot cleaned frequently; lights near dishwasher and in walk-in cooler not shatter resistant; multiple not functioning pieces of equipment in kitchen.
