Wood County inspections: One restaurant receives 12 violations and 10 ace reports

The following are the latest results of Wood County Health Department inspections on licensed food establishments and other types of businesses, which the Wisconsin Rapids Daily Tribune and Marshfield News-Herald report monthly.

Auburndale

  • Subway No. 121, 6018 Main St.: March 2: No violations.

Marshfield

  • Abares Outerlimits, 9850 Wood County Y: March 29: One violation: No certified food manager for establishment.

  • American Legion No. 54, 2100 S. Maple Ave.: March 7: No violations.

  • Blue Heron Brew Pub, 108 W. Ninth St.: March 23: Two violations: Sliced ham in top cooler drawer not at proper temperature; walk-in freezer not in good repair.

  • Pizza Ranch, 1302A N. Central Ave.: March 22: One violation: Walk-in freezer not cleaned on frequent basis (Corrected during inspection).

Nekoosa

  • Sacred Heart of Nekoosa, 711 Prospect Ave.: March 14: No violations.

Vesper

  • JoJo's, 6580 Cameron St.: March 23: No violations.

Wisconsin Rapids

  • Bull's Eye Country Club, 2800 Ridgewood Trail: March 22: Five violations: Gravy, pasta, tartar sauce and blue cheese in walk-in cooler exceeded date mark; no single-use toweling or hands-free drying device available at handwashing sink; sanitizer bottles not labeled; racks in reach-in coolers rusty and not easily cleanable; no irreversible registering thermometer.

  • Chips of Wisconsin Rapids, 1330 W. Grand Ave.: March 13: Three violations: Shredded lettuce not at proper temperature; working containers of degreaser not labeled; handwashing sink in women's restroom not working correctly.

  • Cobblestone Breakfast, 3010 S. Eighth St.: Feb. 28: No violations.

  • Culvers of Wisconsin Rapids, 2711 Eighth St. S.: March 21: No violations.

  • El Mezcal, 930 Kuhn Ave.: March 30: Three violations: Raw shrimp stored over diced tomatoes in walk-in cooler; shrimp improperly stored (corrected during inspection); test kit not available for checking sanitizer solutions.

  • Hoozels, 4010 Plover Road: March 28: No violations.

  • I Spoon Sushi, 4531 Eighth St., Suite 106: March 30: 12 violations: Pasta salads and cut melon on salad bar not at proper temperatures; employee washing hands in unapproved sink; shrimp improperly thawed; improper cooling methods for cooked chicken; sushi rice not properly marked with a discard time; drinking cup without cover in food preparation area; flour, salt, sugar, cornstarch and rice stored in unlabeled containers; scoops without handles stored in sugar, flour, salt and rice; can opener in kitchen dirty; reach-in cooler in sushi bar and reach-in cooler on the cooks' line not equipped with permanently attached thermometer; exterior door in kitchen left open; no handwashing signs at sushi bar and kitchen handwashing sinks.

  • Ida's Neighborhood Sports Bar and Grill, 1981 Lincoln St.: March 15: No violations.

  • The Junkyard Bar & Grill, 9047 State 13 S.: March 20: No violations.

  • Misty's Menu, 9041 State 13 S.: March 28: Four violations: Sanitizer in dish machine not used according to instructions (corrected during inspection); scoops without handles stored in food containers on the cooks' line reach-in cooler; boxes of food on floor in the freezer; deflector shield on the ice machine and racks above the cooks' line reach-in cooler soiled.

  • Taco Johns, 610 E. Grand Ave.: March 22: No violations.
