Wood County inspections: One restaurant has 7 violations and seven ace reports

The following are the latest results of Wood County Health Department inspections on licensed food establishments and other types of businesses, which the Wisconsin Rapids Daily Tribune and Marshfield News-Herald report monthly.

Auburndale

  • Kizzy and Sue's Bar and Grill, 5788 E. Main St.: Feb. 1: No violations found.

  • St. Mary's Catholic Church, 5866 Main St.: Feb. 1: No violations found.

  • Wonder Inn Willys, 5778 Main St.: Feb. 1: No violations found.

Marshfield

  • Culvers of Marshfield, 109 W. Upham St.: Feb. 21: One violation: Ice scoop stored on rack that is not cleaned every four hours (corrected during inspection).

  • Elk's Lodge No. 665, 113 E. Second St.: Feb. 16: Two violations: No consumer advisory on menu; test kit not available for checking sanitizer concentrations.

  • Holiday Inn-The Rivers, 750 S. Central Ave.: Feb. 8: Seven violations: Shrimp stored below burger (corrected during inspection); drawer cooler not holding tomatoes, cheese, peppers and mushrooms at proper temperature; no procedures for responding to vomiting or diarrheal events; salmon improperly thawed; no certified food manager for establishment; no food protection manager's certificate posted in establishment; walk-in cooler door and gasket soiled.

  • Marshfield Medical Center Four Seasons Cafe, 611 St. Joseph Ave.: Feb. 16: One violation: Floor below soda coolers dirty.

  • McDonald's, 1101 N. Central Ave.: Feb. 7: One violation: Handwashing sink operating incorrectly.

  • Nutz Deep II, 801 S. Central Ave.: Feb. 8: One violation: No procedures for responding to vomiting or diarrheal events.

  • Royal Tokyo Steakhouse, 112 First St.: Feb. 21: One violation: No handwashing signs at handwashing sink in preparation area.

  • The Spot, 500 N, Central Ave.: Feb. 21: One violation: No handwashing sign at women's restroom handwashing sink.

  • Subway No. 118, 611 St. Joseph Ave.: Feb. 7: No violations found.

Nekoosa

  • West River Cafe, 1010 Market St.: Feb. 7: Three violations: Working containers of degreaser solution not labeled with common name of product (corrected during inspection); boxes of food product stored on floor in walk-in cooler and freezer; floor in walk-in cooler rusted and not smooth and easily cleanable.

Wisconsin Rapids

  • Aaron's Wines and Steins, 160 First St. N.: Jan. 31: Three violations: Ranch and marinara stored in unlabeled containers; no irreversible registering thermometer for measuring utensil surface temperatures in ware washing machine; cutting board on cook's line scored.

  • B's Tap House, 140 E. Grand Ave.: Feb. 16: Three violations: Test kit not available for checking sanitizer concentrations; no certified food manager for establishment; drink cup without cover seen in food preparation area.

  • Boones Saloon, 8010 State 13 S.: Feb. 21: Two violations: Raw beef stored above coleslaw in walk-in cooler (corrected during inspection); crate of potatoes stored in handwashing sink.

  • The Buck Rub of Wisconsin Rapids, 1511 N. First St.: No violations.

  • Jammers, 2810 Eagle Road: Feb. 21: No violations.

  • St. Lawrence, 530 10th Ave.: Feb. 17: No violations.

  • Tropical Smoothie Cafe, 1951 Eighth St. S.: Feb. 2: One violation: Wall across from ice machine damaged.

  • Wood County Jail, 400 Market St.: Feb. 10: Two violations: No certified food manager for establishment; no food protection manager's certificate posted in establishment.
