Wood County inspections: One restaurant gets double-digit violations

The following are the latest results of Wood County Health Department inspections on licensed food establishments and other types of businesses, which the Wisconsin Rapids Daily Tribune and Marshfield News-Herald report monthly.

Arpin

  • Dupe's Bar and Grill, 6328 Main St.: Jan. 18: No violations.

Marshfield

  • Burger King No. 5278, 1015 N. Central Ave.: Jan. 16: One violation: Storage drawer not cleaned frequently (corrected during inspection).

  • Casa Amigos Mexican Restaurant 2, 12 N. Central Ave.: Jan. 18: One violation: Fish improperly thawed (corrected during inspection).

  • Little Caesar's Pizza, 408 N. Central Ave.: Jan. 9: One violation: No certified food manager for establishment.

  • The Neighborhood Bar, 600 W. Blodgett Ave.: No violations.

  • Southway Pub and Grub, 1212 S. Central Ave.: Jan. 18: Two violations: No approved food manager for establishment; abundance of cardboard and paperwork in kitchen.

  • Swamp Bar, 511 S. Central Ave.: Jan. 18: No violations.

  • T-Bones Bar & Grill, 400 N. Central Ave.: Jan 16: No violations.

Nekoosa

  • Nekoosa Cafe, 324 Market St.: Jan. 10: Three violations: Chorizo in prep cooler past its date mark (corrected during inspection); improper cooling methods used for sausage gravy (corrected during inspection); no handwashing sign at kitchen handwashing sink.

Wisconsin Rapids

  • American Family Table, 1530 W. Grand Ave.: Dec. 29: Eight violations: Cooked potatoes not held at proper temperature (corrected during inspection); soap not available at kitchen handwashing sink; cardboard used to line shelving throughout kitchen; shelving on both sides of dish machine and below microwave rusted; no attached thermometer in reach-in cooler; cooking equipment on line not cleaned frequently; floors below equipment not cleaned frequently.

  • Checkers Restaurant, 1840 Eighth St. S.: Jan. 18: Two violations: Wall and floor juncture in dish area not coved; dust accumulation on ceiling tiles above cooks' line.

  • Domino's Pizza, 2880 Eighth St. S.: Jan 26: One violation: Test kit not available for checking sanitizer concentration.

  • First Street Pub, 1451 First St. N: Jan. 30: No violations.

  • Four Stools Short, 180 N. Second St.: Jan. 4: One violation: Handwashing sink not hands free.

  • Hollyrocks, 114 Second St. N.: Jan. 10: No violations.

  • Jennings and Co., 210 First St. N.: Dec. 29: Three violations: Coleslaw, salsa, chowder and BBQ sauce past their date marks (corrected during inspection); portion cups without a handle stored in contact with food; multiple boxes of food stored on floor in walk-in cooler and freezer.

  • Johnny's, 2610 Eighth St. S.: Jan.10: One violation: Iodine test kit not available for checking sanitizer concentration.

  • Olympic II Family Restaurant, 2520 Eighth St. S.: Jan. 17: Six violations: Pecan pie and tapioca in dessert cooler not at proper temperature; roast and meatloaf in walk-in cooler past its date mark (corrected during inspection); bleach not being used according to label instructions (corrected during inspection); improper cooling methods used for chicken gravy and roast; cardboard used to line shelves and floor in kitchen; dust accumulation on ceiling above dish washing area. Re-inspection Jan. 18: Five violations: Roast and meatloaf in walk-in cooler past date mark (corrected during inspection); bleach not used according to label instructions (corrected during inspection); improper cooling methods used for chicken gravy and roast; cardboard used to line shelves and floor in kitchen; dust accumulation on ceiling tiles in dish room.

Re-inspection Jan. 18: One violation: Ice accumulation in walk-in freezer.

  • Pizza Hut, 2001 Eighth St. S.: Jan. 17: Two violations: Ammonia test kit not available for checking sanitizer concentration; no irreversible registering thermometer

  • Portesi's Fine Foods, 4110 Eighth St. S.: Jan. 25: Two violations: No test kit to monitor sanitizer concentrations; no irreversible registering thermometer in the hot water mechanical warewashing operation.

  • Sportsman's Pub, 4810 Eighth St. S.: Jan. 16: Two violations: Bleach test kit for checking dish machine sanitizer concentration not working; wash tub stored in hand sink.

  • Square Bar, 210 Johnson St.: Jan 25: No violations.

  • Subway No. 131, 3810 Eighth St. S.: Jan. 10: One violation: No handwashing sign at handwashing sink.

  • Tamaracks Pub, 481 Ruby Ave.: Jan 26: No violations.

  • Walmart Subway, 4331 Eighth St. S.: Jan. 23: One violation: Coffee bags stored below hand sink (corrected during inspection.)

  • The Whitney, 451 Grand Ave.: Jan. 18: 11 violations: Chicken, heavy cream and cream of mushroom soup not stored at proper temperature; establishment does not have procedures for vomiting or diarrheal events (corrected during inspection); salmon improperly thawed; consumer advisory missing disclosure statement; water for sanitizer rinse in warewashing machine not at proper temperature; ice accumulation in ice cream freezer; no irreversible registering thermometer for hot water in warewashing machine; salamander on cooks' line not cleaned frequently; floor and wall juncture throughout kitchen not coved; tile on wall missing in the dish room; multiple nonfunctioning pieces of equipment in kitchen.
