  • The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility, as evidenced by this non-compliant inspection.

  • Refrigerated ready-to-eat, time/temperature control for safety foods, (cut vegetables and egg rolls) prepared in the food facility and held for more than 24 hours, located in the walk-in cooler, is not being date marked.

  • The handwash sink located in the employee toilet room does not have water at a temperature of at least 85 degrees Fahrenheit, Employees must use the public toilet room until hot water is restored.

  • Food facility inspection indicates evidence of insect activity in kitchen and ware washing areas. Facility does not have a pest control program. Observed at least one dead roach-like insect under the slicer table. Observed one live adult roach-like insect at the open steel splash plate wall next to the three-basin sink.

  • Food facility is not removing dead pests as evidenced by observed old/decaying cockroach-like insects on the floor in the kitchen area.

Violations:

  • The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility, as evidenced by this non-compliant inspection.

  • Observed 12 medication bottles stored on a shelf with food items near the floor dough mixer in the kitchen area. Corrected.

  • Observed many employee's beverage containers on prep tables and on metal racks in the kitchen, a food preparation area.

  • Observed a food employee prepping food and pizza and not wearing a hair restraint.

  • Observed all food shakers and bottles not being labeled with the common name of the food. Also, all bottles and shakers observed with accumulation of food build-up on the exteriors and in need of cleaning .

  • Observed wiping cloths in extremely unclean condition in back area on the shelf next to the floor dough mixer. Corrected.

  • Observed wet wiping cloths in the front and back areas, not being stored in sanitizer solution.

  • Observed large two-door Baine Marie cooling unit used as a pizza station and small Cold Tech two-door Baine Maire cooling unit used for a sub-station, were inoperable, in the front counter area.

  • Observed numerous food containers in the top of the two Baine Marie units in the front counter area being held at 56 to 71 degrees, instead of 41 degrees or below, as required. Corrected.

  • Commercially processed refrigerated sliced meats and cheeses, ready to eat, time/temperature control for safety food, located in the walk-in cooler and small glass front cooler in the kitchen/prep area and held more than 24 hours, is not being marked with the dates it was opened.

  • Sponges observed in the back area being used to clean food contact surfaces. Corrected.

  • Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration.

  • Observed three-bay sink not set up for wash, rinse and sanitize. Small tub in the middle set up for wash and third sink with clear water and no sanitizing step.

  • A knife located on the shelf next to the floor dough mixer, with silver tape around the handle and a portion of the blade and not in good repair or condition. (Corrected). Observed a heavy accumulation of static dust on the hood vents over the stove in the kitchen, with the possibility to contaminate foods being cooked below.

  • Observed the following on food contact surfaces: Slicer with accumulation of food on the underside of the blade and not clean to sight or touch. Manual can opener with accumulation of dried food on the blade and blade holder and not clean to sight or touch. A gray bus tub filled with lids, with a heavy accumulation of food debris in the bottom and contaminating lids stored inside. (Corrected.) A black mold-like substance on the drainboard mat used to dry clean dishes on and coming in contact mat used to dry clean dishes on and coming in contact with the dishes/utensils cleaned. A black mold-like substance on the interior of the ice machine. Machine also had assorted drinks stored inside contacting the ice. Two wooden pizza paddles stored directly between the two ovens, not a clean surface, instead of on a wire provided on top of the oven. (Corrected) Large knives and assorted utensils stored in the top bread drawer with batteries, screw drivers, crumbs and dirt.

  • Observed underside motor housing and splatter guard with accumulation of dried flour and dough on a non-food contact surface, with the possibility to contaminate dough being stored below.

  • Numerous boxes of "employee" items through-out the facility, in the walk-in and on various shelves being stored with facility foods and equipment.

  • Food facility has a certified food employee, but a certified food employee was not available during all hours of operation, as evidenced by an inability to talk with the certified food employee during this inspection.

  • Observed ServSafe Certification that expired on October 20, 2022.

Violations:

  • The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility, as evidenced by this non-compliant inspection.

  • Food employees observed in kitchen area, not wearing proper hair restraints, such as nets, hats, or beard covers.

  • Observed waffle fries, tater tots, and sweet potato fries stored directly in deep freezer with no protective covering and ability of foods to touch ice/condensation.

  • Observed BIB's and food packaging stored directly on the floor in backroom storage area, rather than 6 inches off the floor as required.

  • Food facility does not have available sanitizer strips or test kit to determine sanitizer concentration.

  • Interior top of microwave, can opener blade, and interior top of dishwasher, a food contact surface, was observed to have food residue and green yellowish matter, and was not clean to sight and touch.

  • Observed single-service, single-use articles stored in front counter area which were not in the original protective package or inverted.

  • Handwashing sink in the kitchen area is slow to drain indicating a clog and needs to be fixed.

  • Floor drain at the dishwasher and three-bay sink area with accumulation of black matter (slime) and needs to be cleaned.

  • Hood filters observed with accumulation of grease and in need of cleaning.

  • Spray bottles of blue and clear liquid, one in the kitchen area and one in the front counter area, with no common name label. Spray bottle of blue liquid marked cleaner sanitizer that is a pink liquid is mislabeled.

Violations:

  • The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection.

  • The Person in Charge did not demonstrate adequate knowledge of the PA Food Code as evidenced by incorrect responses to food safety questions related to approved suppliers.

  • Food Facility Person in Charge not able to provide documentation that raw whole frozen chicken, raw froze rooster, and raw frozen beef foods are from an approved source. Operator stated they buy the raw refrigerated meat in cases, repackages and freezes at the facility. The invoice was a picture on the PIC's phone. PIC was unable to provide original paper copies of the invoices. These frozen meats cannot be sold at the facility and must be removed from the premises.

  • Food facility has bamboo shoots repackaged in baggies at the facility. Invoices not available. Foods are not labeled or in original packaging. These foods may not be sold in this manner in the facility.

  • Observed two dozen bulk bags of rice and other bulk dry goods stored directly on the floor in the storage room area, rather than 6 inches off of the floor, as required. Cases of open raw vegetables stored directly on the floor in the retail area.

  • Observed raw frozen food unwrapped and unprotected in the chest freezer which is subject to potential contamination.

  • Trash receptacle/waste handling unit outside does not have a tight-fitting lid or cover, is over-filled, and trash is spilling onto the sidewalk.
