Violations:

  • The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility, as evidenced by this non-compliant inspection.

  • An open employee's beverage container was observed on pizza prep table, a food preparation area.

  • Severely swollen, distressed canned anchovies packed in oil were observed stored in unplugged cold hold unit and intended for use or sale in the food facility. Corrected, Facility discarded.

  • Observed wet wiping cloths throughout facility, not being stored in sanitizer solution.

  • Marinara sauce was held at 78 degrees Fahrenheit on the stove top, rather than 135 degrees or above as required. Corrected. (Sauce was reheated to 165 degrees.)

  • Contents of sandwich making station was held at 58 degrees Fahrenheit o above rather than 41 degrees or below as required. Corrected. (Food was discarded. Facility will not use this unit until it has been repaired and is working properly.)

  • Thermometers for ensuring temperatures of food are not available or readily available.

  • Food facility does not have available sanitizer test strips or test kit to determine appropriated sanitizer concentration.

  • Observed deeply scored and stained cutting boards not resurfaced or discarded as required.

  • Temperature measuring devices for ensuring property equipment temperatures are not calibrated and/or functioning properly in sandwich station refrigerator equipment.

  • Slicer, a food contact surface, was observed to have old food residue and was not clean to sight and touch. Interior top of microwave, a food contact surface, was observed to have old food residue and was not clean to sight and touch. Soda nozzles, a food contact surface, was observed to have pink mold-like residue and was not clean to sight and touch. Interior of ice machine, a food contact surface, was observed to have black mold-like residue and was not clean to sight and touch. White pizza dough pans and numerous other food pans, a food contact surface, was observed to have old food residue and was not clean to sight and touch.

  • Observed black mold-like residue on the interior doors, floors, walls and shelves of cold hold unit at the pizza-making station, a non-food contact surface.

  • Observed hoods above fryer and grill area, with an accumulation of grease on non-food contact surfaces.

  • Observed light fixture above pizza oven where pizza paddles are stored, with an extreme accumulation of dust on non-food contact surface.

  • Paper towel dispenser was broken and paper towels did not dispense at the handwash sink in the kitchen area.

  • Mops are not being hung to air dry.

  • Observed one can of Raid being stored on the same shelf as pizza pans. Corrected.

  • Food facility using rodent bait placed in bait stations throughout kitchen and dry storage areas which are not covered and tamper resistant. Corrected (Facility removed bait stations.)

  • The Perso in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection.

  • Food facility is offering for sale, Floyds on the go CBD gummies (Red Raspberry Strawberry) that contain an unapproved additive as specified in 21 CFR 170-180 relating to food additives. Such products are under FDA regulation and, at which time, CBD is not recognized as a safe substance to be added to food.

  • Observed raw chicken in the walk-in cooler stored above energy drinks.

  • Pre-made lunchmeat and cheese sandwiches, a refrigerated ready-to-eat time/temperature control for safety food in the GRAB and Go area, was not compliant with date marking by being labeled with a discard or use by date of not more than 7 days, and requires discarding.

  • Tongs utensils, located on the hot dog grill area, are not in good repair or condition.

  • Interior of microwave, soda fountain nozzles, and slicer, a food contact surface, was observed to have food residue was not clean to sight and touch.

  • Door to the back room has an accumulation of residue where everyone touches it.

  • Observed black residue on cooler glides and white racks in the walk-in cooler and in need of cleaning.

  • Observed row of cabinets behind the front counter area are in need of cleaning, dirty dusty with black residue.

  • The handwash sink in the Kitchen area was blocked by plastic lids and bucket and not accessible at all times for employee use. Corrected.

  • Observed wall in the kitchen area with a hole and in need of repair, also dry wall damage to wall in the back storage area that needs to be repaired.

  • No sign or poster posted at the handwash sink in the bathroom area to remind food employees to wash their hands.

  • Walls and floors in the fryer area have accumulation of grease and food debris and in need of cleaning.

  • Observed bleach and bathroom cleaner stored in a cabinet on a shelf above paper drink cups for customers. Corrected.

  • The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection.

  • The Person in Charge did not demonstrate adequate knowledge of the PA Food Code as evidenced by incorrect responses to food safety questions related to plumbing air gap, time in lieu of temperature, and date marking ready-to-eat foods.

  • Prepared sauces in the walk-in cooler area stored open with no covering.

  • Raw shell eggs are stored directly above ready-to-eat sauces (uncovered) inside the walk-in cooler.

  • Refrigerated ready-to-eat, time/temperature control for safety food (egg rolls, breaded and fried meats, cut vegetables) prepared in the food facility and held for more than 24 hours, located in the walk-in cooler.

  • Time in lieu of temperature being used in the food facility to control ready-to-eat potentially hazardous foods (egg rolls, cooked meats, cut vegetables) without written procedures or documentation to verify disposition of food. Raw chicken wings in marinade were 70 degrees Fahrenheit, three cases of raw pork were 57 - 61 degrees. Provided training.

  • The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection.

  • The Person in Charge did not demonstrate adequate knowledge of the PA Food Code as evidenced by incorrect responses to food safety questions, related to proper cooling, waste water disposal, hand washing, hand sink use and requirements, protection of foods from contamination, time in lieu of temperature, and ware washing.

  • A Food Employee was observed washing their hands at the three-basin sink rather than the designated handwash sink.

  • An open employee's beverage container was observed on shelving over preparation table.

  • Observed in-use utensils stored over the handle of a bus cart where it was exposed to contamination from the cart and employee clothing.

  • Single-use plastic bowls are being used as dispensing utensils in bulk dry foods and sauces.

  • Observed wet wiping cloths in preparation area, not being stored in sanitizer solution.

  • Raw chicken (three five-gallon buckets and four ten-pound bags) food was held at 48-57 degrees Fahrenheit rather than 41 degrees or below, as required. Meat was being cut for marinating and cook. Provided training. (Smaller amounts should be removed at a time for preparation.) Foods were placed in walk-in cooler to chill below 41 degrees within three hours.

  • Refrigerated ready-to-eat, time/temperature control for safety foods (cut vegetables and egg rolls) prepared in the food facility and held for more than 24 hours, located in the walk-in cooler, is not being date marked.

  • Condensate drip lines in the walk-in refrigeration is not sloped to allow for proper drainage of condensation. Foods are stored directly below two condensate line leaks. Two vessels used to capture leaking condensate are filled with slimy water and in direct contact with foods.

  • Observed food employee using manual warewashing sink as a handwashing sink.

  • Observed food employee using two basins of manual warewashing sink for food preparation/thawing while, simultaneously, the third basin was observed to have dish washing solution/dirty dishes and without sanitizing between uses.

  • Foods (diced vegetables - egg roll filling) piled high on preparation table in contact with table wall seam and back splash seam that have visible dirt in all crevices. Food should be discarded.

  • Observed sides and interior of cooking equipment, in the cooks line area, with encrusted grease and soil accumulation.

  • Walls, ceiling, floor and fixtures inside the walk-in cooler are extremely dirty and in need of cleaning.

  • Observed shelving equipment, inside the walk-in cooler, with an accumulation of dust, dirt food residue, debris, on non-food contact surfaces.

  • The handwash sink located in the employee toiler room does not have water at a temperature of at least 85 degrees Fahrenheit. Employees must use the public toilet room until hot water is restored.

  • Old food residue, dishes, and utensils observed in the handwash sink in the food preparation area.

  • Plumbing system not maintained in good repair - observed three-basin sink leaking at the waste water pipe(s).

  • Wastewater from the food facility is not being disposed of properly to an approved sewage system, dirty mop water is being disposed of in the storm drain located in the alley behind the food facility. Provided training (Operator may dispose of waste water in the stainless steel sink located to the left of rear exit door, providing this sink is the only one used for waste water disposal.

  • Flood in the kitchen/storage/preparation area is made of laminate tile, tile is not sealed to the floor, is cracked/roughened in several areas and is not cleanable.

  • Floor and walls in the kitchen/ware washing/food preparation/food storage areas are cracked/roughened/and are not smooth, cleanable surfaces.

  • The floor/wall juncture in three basin sking area is not coved adn closed to 1/32 inch.

  • Walls throughout kitchen area have holes, or are broken and in need of repair.

  • Food facility inspection indicates evidence of insect activity in kitchen and ware washing areas, facility does not have a pest control program. Observed at least eight dead roach-like insects in the three basin sink area and one dead roach-like insect under the hand sink. Observed two live adult roach-like insects at the cove base under the three basin sink and one live adult roach-like insect on the rolling tray cart beside the walk-in cooler.

  • Food facility is not removing dead pests as evidenced by observing old decaying cockroach-like insects on the floor under the sinks and equipment n the kitchen area.

  • Floor under equipment in the kitchen and food preparation area is extremely filthy and in need of cleaning.

  • Heavy accumulation of oil and static dust on ventilation hood and ansul piping over the wok station.

  • Employee toilet is not being cleaned as often as necessary to keep it clean.
